It tends to fundamentally affect the story for one, since pretty much no human being enjoys playing second fiddle to the Chosen Protagonist. To a degree it can also affect the presence of NPC companions (the most acclaimed part of BioWare games at this point) since few people have patience to sit around while another person clicks through dialogues with the NPC drone.
Baldur's Gate might have worked slightly differently when it comes to these but that's been ages ago and people's expectations changed since when it comes to their own role in a game. And in addition it should be rather clear at this point BioWare isn't taking their design ideas from that old chestnut of the game anymore, so you likely shouldn't expect anything like that.
