If you unload all AE immediately on my version, the cooldowns on your spells may well be down when the second and third waves come, and you're going to be screwed, because you won't be able to quickly eliminate the weaker enemies. It sounds, from the fact that you're talking about reloading and then acting accordingly like you're actually doing the same thing, but saying you're not. If you're having to position to deal with waves of enemies, then that's tactics, my friend. I do agree that it should be clearer where they're coming from though!

If you don't use doorways and so on, it's much harder to fight effectively. I get the camera complaints, but pretending you *can't* use doorways etc. because of the camera is just silly. I do it all the time, and somehow I don't end up staring at the ceiling and floor. So maybe there's something wrong with yours?
As for boss fights, well, yeah, how you spec affects them. Sure, if you maximise AE at all costs, that will make lots-o-mobs fights easier, and bosses harder - it's your choice, ultimately.
Well my main AoE spell has been Tempest. It lasts 20 seconds and has a 30 second cooldown. So there is only 10 seconds gap between it ending and me casting it again. I can then rotate any other AoE spells (Chain Lightning, Fist of the Maker, Fireball, Firestorm, whatever you like) around casting it along with using Petrify, Crushing Prison and single target spells. I have overexaggerated how often I reload
The doorways I had problem with were mainly inside houses. Try it inside the Fenris house map (which you fight in a several times). If you try to pull the enemies outside of the main rooms into small side rooms they all have low ceilings and you can't see a lot other then the controlled character and maybe an enemy right in front of you. You certainly can't target AoE spells accurately, as you can only target the enemy right in front of you. If you get pushed back into a wall (where you can often get stuck) the viewpoint becomes even worse. There doesn't seem to be anyway to get into a first person view so the controlled charcter isn't blocking the screen. Other areas aren't too bad, but then I have found it much easier to just run as far away as possible from the initial point of combat stopping the waves from overwhelming me in hard fights.
The AoE spec has caused me problems when I fought against single enemies, but so far I have had one fight against a single opponent (I haven't gotten very far into Act 2). Every other fight has been against multiple enemies, even Elite Boss fights have waves of adds that need to be AoE'd as fast as possible.
