The first romance I ever experienced was in "Final Fantasy IV". The relationship between Cecil and Rosa was fairly stereotypical and sappy--not to mention uninteractive, but I still count it. If you are asking for interactive romances, my answer is Jaheira from "Baldur's Gate II". Although I did not complete it that time since I abandoned the playthrough somewhere in the Underdark.

Probably Meryl in Metal Gear Solid and it really pulls at your heartstrings if you submit to the torture and end up going with the Otacon/Stealth Camo ending.
