So I've reached the end of the game, just before you begin the last quest (The Last Straw). I've given Merrill her two gifts, did her required companion quests, have 100% friendship, and still I cannot initiate the " Friend or Foe" dialogue that cements her companionship. Is anyone else having this issue? If you did get the final dialogue, any input on how? Thanks!

Did you initiate the dialogue in the Gallows Prison or Gallows Courtyard?

I had the same problem during my playthrough. I went ahead with the final quest, and in the "calm before the storm" opportunity to chat with your allies, Merril and my character kissed and had the lovey dovey talk (accompanied by an achievement for completing a romance). My guess is the questioning beliefs talk is not essential to cementing the romance, though I think her companion quest is.
