Gaining Maximun Friendship Levels With All The Characters?

 So I'm starting my second playthrough, and I'm wondering how to gain max friendship levels for each character. Aveline, Merrill, Verric, and Anders I got just about maxed out, but Fenris, and Isabella and I had some difficulties with. Are the certain quests that provide high approval levels should you choose certain options? 
or certain dialogue options that are best to choose? What did you do?

If you are trying to max out all what you will probably have to do is change out your party members once you have their full friendship. Isabel likes witty remarks and not outright agreeing to quests (get compensation first).

I'm in the middle of Act 2 and have managed to get full friendship with all the characters save Carver. I didn't really even try.

In general, be caring, nice, and helpful. Let them make up their own mind when it comes to things.

As I played a mage, I avoided taking Fenris along for regular questing for RP reasons but supported his decision to kill the magister's apprentice. I told him giving his word to her and then killing her was wrong, however, and he seemed to agree.

I have no idea how I managed to get on Isabela's good side. At first she was annoyed with the various charitable/law-abiding acts I performed but suddenly Isabela thought I was great. The turning point seemed to be the Fade + joking with her when she betrays you (got +10 approval right as she attacked me) + forgiving her afterwards.

I think sleeping with her helps.

I hardly ever used the middle option and often rejected rewards or agreed to help people right away.

Well I got my friendship with Isabel fairly high and I still had to fight every one, not just the big bad boss man. Any help would be appreciated.  P.S. Forgot to add that she didn't come back even after I let her take the book and run.

No need to raise this treat from the dead for that, but ok.
Isabela only comes back at the end of act II if you had her "Questioning beliefs" talk. In act II, these require 50% friendship or rivalry. If you really want to duel the Arishok, you could also bring Fenris, he can arange a duel. Be warned, the duel isn't really easier than fighting everyone. DA combat just isn't made for duels.

I think you have to setup characters with similar personality together and once you max out their friendship, you swap them out,
