Here are all of the armor upgrades and companion gifts available for all your companions, as well as the Armor sets for Hawke, sorted by Act and where you find them. Let me know if I missed any, and I'll update the list.

Act 1:

Quest: Birthright
Bethany/Carver Upgrade (Heirloom Amell Protective Sigils/ Fereldan Girded Plating)
Bethany/Carver Gift (Portrait of Your Mother/Torbius’ Documents)
 - Both are found in seperate chests in the basement area of the Amell Family Estate

Thanx for this, I was trying to understand why my second playthrough Aveline was not fully armor upgraded when I pick EVERYthing EVERYwhere. *sigh*

There appears to be an issue with her Act 3 upgrade. On my most recent playthrough, it did not drop from Jeven. No idea why. It appears to be bugged.

Recent, I'm on 1st nightmare playthrough after 2 normal and 1 hard. Try never dropped for me off jeven

I cant find Sebastian's Protection of the Faith, i looted all templars i killed and none of them had it.

/edit: it says i have 2 upgrades on him but the last one is mail undercoating, if the one listed is the last one he equipped.

Even more frustratingly, why did they make it so that you can't fully uograde Varric unless you had an Awakenings import in which Nathaniel survived... and then mess up the first release of the game so that it STILL didn't give you the Nathaniel quest even when he DID survive (i.e. import game flag not recognising him as alive unless he was physically in your final party that went after the Mother)? Grrr...
