One of my companion quests was to go talk to Merrill ("A Talking To"). When I approached her she was doing the crying bit in front of the mirror and talking about how "she's gone" and "she can't go back to the Dalish or they'll kill her". After that point the mirror disappeared. When that scene ended I talked to her again and the conversation with Varric from "A Talking To" was going on. At one point she referenced the mirror which wasn't there anymore.

Sketchy on the Details - After encountering assassing in at the Docks, I have yet to encounter any more. I thought I've searched everywhere, but I encounter no more assassins.
Blackpowder Courtesy - This one has stopped me from advancing the game. I have to speak with the Viscount again informing him of the Arishock's reaction. The Keep is flagged with two current quests on the Kirkwall map: this quest and Aveline's "The Long Road". I enter the Keep and there are no flags on my map. In fact there is no Viscount in his chamber. I can't even activate "The Long Road" with Aveline. The only marker on my map is for "Forbidden Knowledge" which bugged as I indicated above.
Not related to this act, I have on several occasion have a disc read error. I'd play the game fine, then suddenly in an area transition the game would kick me out for the read error. I checked the disc and it's fine. The game loads up, play for 5-10 minutes and bam - hit with the disc read error. Other games work fine, and some evenings the game play without a hitch but since release, there have only been 3 evenings where I haven't had the disc read error. I've got 28 hours into the game!

Seamus even in the Keep either, and I have the Forbidden Knowledge and there is no Tome marker on my map - so I did find the Tome in the Keep!
I'm playing the Signature Edition.

This happened to me as well. Some other related glitches I found relating to this quest:
- The dialogue skips out or just doesn't load against some of the assailant groups. For example, it cut off mid-sentence during the Antivan part.
- During the Hightown attack, all the Denerim assailants had persistant map markers over the positions the entire fight... which stayed on the map after the battle was over.
- Another glitch... just reported to Sketch after the update came on screen, went back to Hightown (Night) and it shows on the map that there are still Denerim attackers.... ran over to the spot and it cleared up.

Isabela's got your back is a sustained ability for when you have her at 100%
friendship. It increases Hawke's and Isabela's attack speed by +5%. This ability
and the Arishock are unrelated (Once you get this ability you can still make
choices in the plot to prevent her from assisting you with the Arishock.)
While it's worded somewhat confusingly due to GUI restraints, this is how it's meant, and it's not a bug.
