End of Act II
Questioning Beliefs, Isabela, quest does not start until after 'To catch a thief' which makes quest impossible to complete as Isabela has already left my party. Result = Isabela leaves party forever.
Forbidden Knowledge, chose to 'Take', quest bugged unable to complete.
Sketchy, Still appears in quest log after completion.

Has anyone seen this glitch??? Went on to do Anders quest "Dissent"... start off in Darktown, Anders asks "are you ready?", get to pick who you take with... then it loads & you're in a cavern w/ your team. But mine is the same exact cavern as the "Blackpowder Courtesy" quest. I even re-found the ship in the bottle to give to Isabela. Get to the end of the cavern & exit out into the Wilderness & sure enough there's Javaris and I'm talking to him again about the crazy elf... go to the "leave area" arrow, look on my map & "Dissent" isn't showing up anymore!
I did save before hand so I can give it another try to see if it works the 2nd time around.

At the end of act II, while confronting the qunari at Vicounts keep, the qunari's horns suddenly dissapeared. At another playthrough the horns lacked texture.

I'm over halfway through my second playthrough (2 quests away from Act 3), and both times, I have been unable to keep Isabela as a companion after I complete her companion quest. I fulfilled all of the requirements to keep her, including promising her she could have the relic.
I noticed during the first playthrough that things got a little flaky around this point. I had a "Questioning Beliefs" notification, telling me to meet up with Isabela at the Hanged Man. I went there right away, and also tried during the day, again at night, etc., until I finally gave up. The quest marker showed up, but Isabela was nowhere to be found.
On the second playthrough, I did things pretty much exactly the same as before (although I can't say with certainty that I did ALL of the quests in the same order). This time, hours later, I still haven't gotten the notification to talk to Isabela. I did find her letter after I promised her she could keep the relic, same as before. Now I'm almost to the Qun quest, and I'm a little worried that I'm going to have to spend yet ANOTHER playthrough without Isabela in the third act. :-(
I had a similar issue with a quest not completing - Sketchy on the Details. I had a pop-up message stating that I had killed all of the gangs, and to talk to Sketch. I did so, but the quest never cleared. I rebooted the XBox, and it actually did clear, finally. Unfortunately, it did not help with Isabela's quest issues.
Edit: Isabela did come back in the second playthrough, hooray! She was at 100% friendship, so maybe the Questioning Beliefs quest was unnecessary? Regardless, I am relieved. :-)

alot of my issues were resolved by going back to gamestop and getting a new disc. had to start a new game though

I just finished the Repentance quest, and Sebastian is locked into my party. I went to the chantry, talked to him, talked to him again to give him his family bow, and I still can't switch him out.

Apologies if already posted, but i've noticed that the only time Merill's "Mourning" quest activates properly is when I leave Bethany at home for the deep roads expedition. Both times when I took her the quest showed as completed before I got it.
Not sure if this happens with Carver as I've only played as non-mage so far.

I am havinf the main character animation slow down. I posted this in the other thread but you guys listed this one as a major issue. My character eventually slowed to a crawl and my character was getting demolished. Went back to a previous save and managed to play the game with my character half as slow. I had to go back to previous saves more than four, its not worth doing a second playthrough if this is not fixed which is sad cause I was greatly enjoying myself until this. Please fix this!

I know some people are saying that the "Mourning" quest glitch with Merrill doesn't prevent romance, but for me, it's completely frozen her out of the game. I get no quests with her at all after that - I completed Blackpowder Courtesy and every quest it was supposed to open up was opened, but Mirror Image was not.
Anyone else having the entire Merrill relationship frozen out by the glitch? Or if you go far enough, is it opening up for you? And if it is opening up, is it opening up after Blackpowder or through some other trigger.
About to trash my save and start again - I have no save from before taking Bethany into the Deep Roads with me (no save before deciding to make her a Warden, sadly), so I feel like my save is utterly useless.

Having a problem with the pick axe quest for the miners at the bone pit. I went to the guy with the pick axes(smith(wokkawokka!)) and talked to him about the pick axes and chose the dialog for him giving me a discount and he said nope and that ended the conversation. Now when i go back to talk to him he just says "Pickaxes?" and my character says"I'll be going now." automatically. Is this just the end of the quest or is there something i'm missing because the smith is still highlighted on my map as a quest coordinate.

to catch a thief-if you enter the lost-end foundry without isabella the event works as if shes there; isabella says 'hes getting away' when she isnt even there, only the voice. Then hawke says 'isabella'

Just a few solutions for some of the problems I've seen:
Forbidden Knowledge: Just so everyone knows, if you read or take any of the tomes, this quest can't be completed. You have to DESTROY all five to learn the location of the last and complete the quest.
For people who have had a character get stuck in their party: My experience has been that if you return to Hawke's home and then leave, the locked character is no longer locked and you can choose your party freely.
For people who are having the slow-down glitch: The glitch is caused by gaining Isabella's friendship and then not keeping her in your party. Every time you load a save without her in your party, you lose another 5% attack speed permanently. Nothing will ever remove this penalty. Neither clearing the cache nor rebooting helps. The same thing happens with Sebastian's friendship ability and damage resistance. There's a thread floating around on here with a more detailed explanation. But basically there are only two ways to avoid it:
1. Do not gain friendship with either Isabella or Sebastian. Try to make them rivals instead.
2. If you DO earn their friendship enough to have their friendship ability kick in (accidentally or not), keep them in your party at all times. Never, ever remove them, and certainly don't save or load without them.
Until there's a patch for this rather severe bug, this seems to be the only workaround.

At the viscounts keep, during the demands of the qun quest i kill all the qunari and what happens is that it loads up a cutscene but jumps back to the beginning where i met merridith the knight commander..
also at bartrands estate, only one of the guards attack and varric is just another member of my party and i dont control him

You need to go back with 15gold to finish the quest.
It'snot clear unless you read the quest description (which I didn't do at first), hence the confusion as usually in a quest like this the npc tells you what you need - Ithink this guy is just an awkward so-and-so.

I'm experiencing a glitch in one of Varric's quests called family matters. I enter Bartrand's mansion, but there is only one guard fighting and he gets killed in one hit. The game is glitched so that I don't see the cutscene after and I cannot continue through the game.
I've tried running around and searching Bartrand's mansion, but nothing happens. I'm pretty much stuck.

When doing the quests to find Hawke's mother, I spoke to the blood mage in the home and kept him alive. I later met him in Darktown as he hinted, had him perform the rite and I went in to complete the quest.
later, while questing, I was in lowtown and saw a boy with a "completed quest" target over his head. My uncle was standing next to him. I spoke with the boy, and it was dialog about how he saw a man faint and my mother stop and ask if he was alright! My uncle continued to press for the whereabouts of my mother - even though I'd already gone through the dialog with him about how the end of the other quest finished. Once the dialogue was done, I received notification that the quest had once again been marked complete.

Twice now I've gotten to the end of Act II with Isabela at 100% Friendship. Once with a platonic relationship, and once with a romance. Of course, she comes back at the Viscount's Keep and saves the day, but after dueling the Arishok, it goes straight into the declaration of a new Champion, to Varric's interrogation, and then to Act III.
I don't get any conversation with her, as the Wiki says I'm supposed to and as she herself alludes to in the first conversation with her in Act III. Her codex entry for "The Last Three Years" also indicates a bug. The entry is repeated within itself, with some minor changes. It starts with "Isabela's return during the Qunari crisis was surprising . . . " . However, at the end of that paragraph, it mentions a conversation with her which mentions her saying, "You're reading too much into it." It then goes on and details her departure and return. The then entry immediately repeats itself. This time, the first paragraph ends with mention of a conversation ending in, "I didn't do it for them. I did it for you. It was always about you." Again, it then has the same paragraph about her leaving and reappearing.
Despite my reliance on information from Act III, it seems the bug is in Act II, so I figure this is still the place to put it.
I also had the issue where Isabela's outfit doesn't permanently update, reverting from her romance-specific to her default armor in a matter of seconds. A Maker's Sigh seemingly fixed that, but I wasn't sure how related it might be.

Enchanter's staff when equipped is upside down. You buy this staff from the Apparel Shop in Lowtown. When equipped as I said it appears upside down. the butt of the staff is up by your head and the top of the staff is down by your feet and it is going into the ground causing clipping.
Current party make-up is Isabela, Merrill, Varric. Level 16 Mage FemHawke. Used potions from Black Emporium. Used Stat enhancing books from various vendors. Currently a Bloodmage/Forcemage combination.

Night Terrors - Act 2 - "Game-Breaking Glitch"
I have read others post about the Night Terrors quest. This quest also posed problems for me. I have had other problem but this one really ruined my experience. I have had problems with dialogue scenes with companions and NPC. They glitch, fade in and out of the scene. Also, both Anders' and Meredith's glowing eyes scenes are messed up; the glowing orbs wander all over their faces. Those were bad enough but when I finished the Night Terrors quest things went downhill fast. I finished the quest without problems, but after the quest I noticed that my mage was reacting/casting spells much more slowly. I thought it had something to do with dexterity and some put points into that with no change. As I continued through the game she only reacted slower and slower. She moved the staff slowly and cast spells slowly. All the other mages in the party were fine, no problems, they fought as before but she only got slower and slower as time went on. By the end of the game, I could cast one or two spells before the fight would be over. I would frequently die because I could not heal myself or move out of the way because she was still stuck in casting mode. This was only a problem during combat, and only with my mage. This is a huge let down for me. I really enjoy the characters and the story and was really looking forward to the final battle, but because of this problem found the battle to only be frustrating and tedious. I am really disappointed by this. I bought both signature editions for my PS3 and my XBOX 360 and was hoping for a better quality product. Don’t mistake me, the story line, characters, and new combat system I REALLY enjoyed, BUT that glitch along with along with the others extremely lessened the game play. Being that both my versions are on console, I am not sure how this will be fixed. I hope it is fixed however, because this has led me to regret purchasing the signature editions. If I new there would be this many problems I would have waited to buy the game, if at all, and spent a lot less money. I sorry for the negative feedback, but I really hope this gets fixed. I am a huge fan and want to get the most out of the game.

I really hoping on a patch soon for the 360 version. I haven't touched Dragon Age II in about a two weeks. My game is virtually unplayable, my main characters save is still stuck in Viscounts Keep, all of my characters attack speed are slowed, I can't start the Exiled Prince in Act 2(main character)and I cant open the extra content chest!! I'm not happy at all right now with Bioware, but I hope I'll be able to play again soon!

I was trying to romance Fenris yet it was Isabella who showed up at my estate for the romance scenario even though I never flirted with her. I turned her down and was hoping that Fenris would still pop in but he never did.

I have researched the Forbidden Knowledge glitch a bit, and it seems that the only way to have the quest clear is to either destroy all of the books, or to take all of the books. If you read one of them and accept the demon's deal to get the level up, which is something that any self-respecting evil character would, you are then unable to go to the final area. I find this to be inherently flawed, because the same benefit granted by accepting the deal can be achieved through the completion of the quest in the evil manner as well, with the added benefits of extra loot, experience, and gold. I guess it is just poorly designed because any evil character will not get the full potential "evil" benefit without knowing exactly what is in store in the quest.

The guide says the consoling words quest is at the beginning of act 2, but I never got any quests called that for her.
