The book, A Slave's Life, does not spawn in the sack in the Alienage at night in ANY act for me. This last playthrough, I saved and checked it and reloaded, and the book is never there. It is always junk loot. Does anyone else have this problem? As far as I know, there is only one sparkling sack in the alienage at night in the game. It just never holds the book. Signature Edition, Xbox 360.
Act 2 Issues (may contain spoilers)

I'm on 360 as well. It seems to me that I had that same thought on my last playthrough. I looked a couple of times at night, and it eventually showed up after I had completed some other quests. There must be a quest completion that triggers it, maybe?
Edit: Now I'm doubting myself. I need to check...
I've noticed that the Brother Genitivi documents (History of the Chantry) are no longer showing up anywhere since the patch, either....unless I'm just really unobservant, which is definitely possible.
Edit #2: I checked on this over the weekend - the book did show up for me in the sack in the alienage at night in Act 2, while I had no active quests in Lowtown.

After completing Inside Job, I left the area and went to the docks to talk to the Arishok. Before going into their compound, I stopped to swap out party members... that's when everything got psychedelic.
The highlight within the party selection screen got huge and solid white. Two companion slots turned grey within the party selection screen, and within the game itself. Poor Sebastian lost everything but his head and his bow. He even lost his hair. I thought it was just a fluke, so I added him to the party. Not surprisingly, he appeared as just a head and a bow. I went to Darktown, and a couple of other places, thinking that maybe a change of scenery would do him some good. Nope. Just a head and a bow.
So...I reloaded from Brekker's hideaway, and tried going somewhere else and choosing my party. Same thing. Then I quit the game, and reloaded that same save. Same thing.
My last resort was to turn off and restart the xbox. As luck would have it, that's what fixed it.
And that's how Sebastian got his hair back.
