Once the game was over Varric clearly said something along the lines of everyone having to leave the Champions side...except Isabella of course.
Problem being Merrill was my main romance in the game. I slept with Isabella first but it was agreed it was just for fun. After that Merill was the main love interest and moved into the estate, she changed to the alternate out fit and before the final battle she and the champion kissed and I got the romance achievement then.
I'm not sure if friend or foe worked properly for me or not to be honest. I'm guessing maybe not as it's not in my list of completed quests.
Not a huge thing as i said, more just worried that it'd affect any save game that I import intoany DLC/expansions that may come out.

In the quest "Best Served Cold," when Merrill is kidnapped, not only was I still able to put her in my party, she was shown laying on the ground in her green outfit - so essentially there were 2 Merrill's in the scene. After the fight, she disappeared from behind me, the green Merrill stands up and says a few words, and then when I regain control, Merrill is standing in front of me in her white outfit and is no longer in my party.
Kind of funny, but also rather annoying.

Dang man, you copied me so well! That is exactly what I did, and what happened to me. I also had the giant quest destroying Merrill bug where she has the post quest conversation first. Now I deleted my guy and all his work and will start over once all the bugs are fixed.
(After Chantry is blown sky high) (what sarcastic Hawke should have said) "That may have been the best fireworks show ever, Anders! .... Oh, Maker-dammit, why was it the chantry? Please hit Uncle Gamlen's house next time.":P

In A Murder of Crows, I got the the cave where the varterral was but couldn't actually engage it. When I ran down the stairs, I hit some sort of invisible wall. I never got to see Zevran.
With Who Needs Rescuing?, it registered in the journal and on the map, but I couldn't start it when I traveled to where I was supposed to go.
The Awiergan Scrolls: Pride Unbound quest also registered in my journal, but I couldn't access it either.
I would also like to add that when I played Act 3, I could see all these quests on my map screen as well as in my journal (just like any other normal quest). However, after a time (and many reloads to try to fix this), they soon disappeared off the map screen though I could still see them as active quests in my journal.
Just to be clear, when I say "map" or "map screen," I'm referrring to the World Map screen where you can select the place to travel and not the map of the "map of the area" you can look at when running around.
Edit: adding--> There was no music playing during the credits.

I didn't have an issue with Murder of Crows but couldn't finish Reining it In along with Rescuing and Scrolls. Frustrating! Good to know you still finished the game and didn't run into a broken plot quest.

Is this an issue that will be addressed with the first round of patches? As I understand it, this glitch can cause her questline to be rendered incomplete. I have just reached this point in the game and if a patch or fix is forthcoming I would like to wait on it before proceeding. Thanks.

Merril cutscenes do not activate in proper order.
Never got the quest involving Nathaniel.
Unsure but at final battle Bethany never arrived.
I have the signature addition, Blood Dragon Armor and it is installed on my elite consol.

I hope this bug hasnt already been posted if so i'm sorry. I killed the high dragon a few levels ago and now I discover I need the dragon's blood crafting material. After doing some research, and correct me if im wrong, the high dragon is supposed to drop the dragon's blood. i went back to the bone pit, after having finished the quest, and i still have quest markers on my map for it. The high dragon's body is still there but i am unable to loot it. any ideas what happend? how can I get the dragon's blood? Thanks in advance for any help.

Especially when they break the quest. I've been doing every quest that I've come across/comes to me and its annoying that some of them cant be finished, let alone started. What's the deal Bioware????

Double Edit: Scratch it all. I cleared my 360's cache and the issue went away.
I have exactly the same problem except that clearing my 360's cache does nothing.
I have completed all active secondary quests. I have completed all companion quests (in that I have helped Anders to get the ingredients he needed and distract the mother), I have completed all side quests I've currently found EXCEPT the Qunari Swords and the Who Needs Rescuing quest (which I can't trigger). I've done the Zevran mission, I've killed the High Dragon, I've even reported back to the herbalist with the fire gland.
There's a golden chevron over the Gallows during the day and that's the only quest marker I have, but when I go there there's no sign of the Knight Marshal and no quest marker in the Gallows.
I'm pretty sure a templar told me she was off overseeing an Annulment the first time I went there?
The letter from Meredith has gone missing from my ticked list of messages, which makes me wonder if this is part of the problem.
Please help, if I can't fix this I think I will have to revert to the beginning of the Act some five hours ago, and that will make me very sad and angry. Am I missing something, or is this a devastating game glitch?
Also, when I clear my 360's cache (new 360, from the Memory option) it briefly flashes an empty progress bar but it never turns green; how can I guarantee my cache has been cleared?
Thanks for any assistance.
UPDATE:
Hoping that it was a matter of not having done enough companion or side-quests or something, I went and did the entire sequence of events leading to the Demon Slayer achievement (killing Hybris), gave Merril the ring from the thief in Sundermount, and killed Hanker, the leader of the Followers of She, in Lowtown. Alas, there is still no sign whatsoever of the Knight Commander in the Gallows and I remain unable to progress the game. Needless to say this is exceptionally frustrating, as I really want the closure of finishing the game after sinking 31 hours into it!
Am I supposed to randomly wander the city looking for more quests to do until I reach an arbitrary limit? I don't get what's going on here.
ANOTHER UPDATE:
Turns out it was a combination of a minor glitch and my stupid error. For some reason there was no quest indicator on the map when I went to turn in the "On the Loose" quest, so I was looking in the wrong place entirely. When the penny dropped and I went to the right place the game worked flawlessly from there (including Merril's quest) and I went on to complete it.
Sorry for doubting you, Bioware. I thoroughly enjoyed the game!

I've been playing as a smart-ass Rogue the whole time. I'm down to the final companion quest besides Merrill's(isn't unlocked yet) and I finish Fenris' quest with his sister and what not. After the quest I realize in the next fight that every single attack I do is in super slow motion. I mean even jumping through the air, my Hawke flies as if she's in a Zack Synder film. I put Haste on her and she only attacks at about normal speed then i.e. how fast she was in the very beginning. So now my Hawke is nigh unplayable.
I'm pretty sure this has been an increasing effect, however, from a negative return on Haste. Through out most of the game most of my battles have started, and ended with Haste up. Whenever I fought without Haste up I always thought that she was attacking slower than normal but chalked it up to just being too use to Haste, so relatively speaking, she seemed too slow. Much too my surprise, though, she always seemed to be getting slower and slower, bit by bit. After that quest with Fenris it all came crashing down hard. So now I've a Rogue Hawke that moves and flies through the air slower than I would of ever believed. This is gonna make me have to end up turning the game to easy just for the final mission at this rate since I'm gonna lose both of my Mages due to the choices I'm gonna make. I was counting on tearing up stuff fast enough with my badass Hawke so just potions would be good enough. It wont be at this rate... I really hope they fix this kind of thing relatively soon. I don't know if it matters, but Anders has been the one casting Haste the whole time.
I've also romanced Merrill, had her on the good path, and gave her the mirror piece and whatnot; and got her conversations out of order as well. She talked about the Keeper being dead, then I told her we'd stay together. I was really confused the first time.
I also sided with the Mages, and in the last battle Carver came to help against Meridith (he was a Templar) and his hair had no textures and was a messed up white.
Varric also mentioned me staying with Isabela to the Seeker after the final battle, but Merrill was my romance in that playthrough. I did, however, do everything for Izzy (companion quests, gifts, and even slepted with her that first time, but told her it was just a fling basically) and had full friendship. I romanced Merrill completely and even had the nice kiss before the final battle.
Edit: Just realized I didn't include the fact that plenty of conversations with Merrill didn't flag right. Through out most of the game when I flirted with her her reaction was the same reaction as if I chose the diplomatic response. (I reloaded the game several times and this was my conclusion) Yet she still visited Hawkes mansion, moved in, romanced to full, etc.
Another Edit: Just remembered. All of the dialouge and what happened with Merrill's cutscene (that was out of order) happened as you'd expect. She talked about dooming her clan because she pursued the mirror and stuff. (I gave her the piece). However, the mirror wasn't there in the cutscene the whole time. It was also just kinda not there after the cutscene. I checked the whole house and everything. I don't know if that's a glitched or if I just missed something, however.
Edit: Something else I just realized. No one seems to remember Isabela getting a new ship because of her quest with Castion or whatever. She herself has mentioned multiple times that she still doesn't have a ship. Other allies keep mentioning it as well. Thing is, she has her ship, and I've even agreed to go with her on it because I'm romancing her. o.O
Yet another edit: I kept Merrill's piece that she needed. So when I got the conversations out of order, she broke the mirror b/c it was the cause of everything going wrong. So now I'm getting her quest to go to the demon so she can try and fix the mirror, thing is, there's no mirror. Just the frame. Haha.
One more Edit: I'm completely convinced now that it has something to do with Haste. I was so slow at the time of orginally writing this that the only way to really even fight was to continue using Haste. This whole time my character has continued to get even slower. My character is so slow with her attacks now that she can only hit about once a second; if even that. Now with Haste on she still hits slower than a normal Rogue Hawke. The weird thing is that now with Haste, she starts out as if it isn't even on her, and she gradually speeds up through out Haste's duration until the fnal hit is about the same speed as normal Rogue Hawke. Mind you this is even with it's upgrade gotten a long time ago. I had used Haste the whole time on my Mage, with me casting it, but I never bothered with it's upgrade until very late in the game. The play through I'm talking about currently, I picked it up with Anders as soon as possible because Haste was just so awesome with a Rogue. So I think it has something to do with the upgrade of Haste causing negative returns or somthing. This whole problem persists whether I am in control of my Hawke or not. It's quite funny to see Hawke basically floating through the air fromt he perspective of another character.
Edit: Even on my second playthrough the sound still messes up in the events that follow the war beginning between the mages and templars after Anders destroys the Chantry. The sound cuts in and out, skips, and does all kinds of strange sounds for the first few encounters.
Another update: I decided to read the codex of the saved game after the game was completed and the codex talks about Merrill pretty much never leaving her house because of how obsessed she is with the mirror. In this play through I didn't romance her, I kept the mirror piece, and she broke the mirror and said she was done with it.
In Isabella's codex the same two paragraphs are repeated twice with the second, second paragraph having a few more things to say that reflect on the romance I had going on with Isabela; though it mentions her saying some things she never said.
I also forgot to make note of the fact that Lelliana never mentioned the romance she had with the Warden in this import's story. Zevran also never mentioned his romance with the Warden in my first game's import.
Also, in the codex about the Warden, it says my Warden in DA:O sacraficed herself to kill the archdemon, then went on to marry Allistar and become queen. Granted I did set the game up with the option of making her queen with Allistar if everything did go right. Though she kinda died... which also makes me just now wonder what was up with being able to play her in all of the DLC...

Some of the last companion armor upgrades are not appearing when you're suppose to get them when you loot someone. Like Jeven for Aveline's armor did not appear for me at all.
I think someone already said something about the quest Who Needs Rescueing. For me nothing appeared at all when I head to The Wounded Coast.
After Merrill's personal quest she's locked in my party, had to head back to the mansion to get rid of her.

The quest where I have to save someone named Orlana from 'somewhere along the Wounded Coast', I went to wounded Coast and searched everywhere but did not find cave or the captured person. I was attacked by the remains of a group of thugs claiming that I had destroyed their group and they wanted revenge. Upon beating the first wave there was a final thug that appeared and I was not able to attack him. I am unsure if the two events are related.

During the first time I fought Meredith at the end of my first playthrough, there came a point where she used some ability to stun everyone on the field of battle. She does this several times, so I wasn't too worried. The problem was, my Hawke (a mage) was off in an obscure corner of the battlefield, and I stayed there, stunned, for more than a full minute (probably closer to two) while Meredith wandered around the field and killed all my other stunned characters, and then finally came over and killed me too. The stun never wore off.
When I reloaded, this didn't happen the second time and I was able to complete the game just fine.
