The Who Needs Rescuing? Quest, NPC is just there and nothing happens.
Merril's stuff doesn't seem to be in order.
Occasionally my companions just freeze and don't fight.
In act 3 the special delivery crate in my house has a plot symbol but I can't do anything with it.
When I finished the game and watched the credits it reloaded me back to before the last big fight (I'm assuming this is a bug, as I though it would take you back to the main menu).

Who Needs Rescuing? is bugged
My import from Origins-Awakening-Golems-Witch Hunt. I get the Fools Gold quest in Act 2 when I had Nathaniel Howe Survive in Awakening. That stops me from getting Varric's last armor upgrade For the Finding Nathaniel quest in Act 3.
If you romance Merril her The New Path quest ending comes b4 even getting the quest. And the mirror disappears from her house too.

Isabela's Regret will not start. shows up in quest log, but she has no plot arrow in Hangman's Pub, will not start converstation.

During my quest to get Zevran for the Antivan Crows, I go into the cave where the Varterral is, and instead of being able to walk into the room and attack the creature, it's like I'm running into a wall and I can't continue further. And if I push more than twice I can't get back up the stairs to exit the cave. It sucks because I can't load to a previous save and when I get out of the cave it locks up on me and the screen freezes. I canNOT continue the rest of my game because the game won't let me. So I'd really hate to have spent 60 bucks on a game the freezes and won't let me continue.
No, Isabella isn't in my group. It's Hawke, Merrill, Anders, and Fenris. Any answer would be great.

-Merrill's quests being out of order. The Conclusion Dialogue in her house for her Act 3 companion quest came at the very beginning of the Act, before I had even done the catching up with her after the time skip chat. This resulted in confusion at first and then spoilers when I finally did obtain her Act 3 companion quest.
-Justice, during Ander's companion quest in the last part where you must go to the chantry, if you have Sebastian in your party when you get to the chantry he adamantly refuses to be a part of tricking the Grand Cleric and leaves the party, at the gather party tool though you can just throw him back in the party with no repurcussions. (Not 100% sure that this was a bug or anything, just seemed a little odd that he would be so adamant about not partaking in Ander's little charade yet if you force him to you don't gain any rivalry with him nor is he actually "unavailable" to bring along)
-Alone, during Fenris' companion quest his sister never appears in the Hanged Man so I am unable to even start this quest.
-Mine Massacre, when you go down to where the High Dragon is and have your Mabari out the little cutscene that shows the dragon landing and creates a little flame barrier preventing you from getting back out will trap your Mabari on the other side forcing you to despawn him and wait on the 30 second cooldown to resummon him so he can help out during the fight. The Mabari will also sometimes glitch out and get stuck in place when the dragon flies off and spawns its adds he seems to be reading the High Dragon as his target still even though she's currently unattackable so he'll ignore all the dragonlings and male dragons beating on him and just sits there until the High Dragon comes back down.
-Who Needs Rescuing?, searched all up and down the wounded coast and never seemed to stumble on any bandits other than the fellow in the NE corner of the map just above one of the entrances who doesn't seem to respond to anything. So I couldn't start this quest.
-Favor and Fault, after doing the dock part and heading to the Gallows with Aveline to speak to Cullen as soon as their conversation ended my system would lock up. It happened about 4 or 5 times in that exact same spot finally one time it just got past it. (Not sure if this was an issue with the quest or my PS3, but it was consistently doing it in the same spot and hasn't locked up anywhere else on me.)
-Rune of Valiance, the description of the rune reads that a piece of armor can only bear one of these runes, yet putting these runes in multiple pieces of equipment has no effect past the first rune applied. So the runes are either not stacking or is it that the description is meant to read that a character can only bear one of these runes throughout all of their armor pieces?
-A Murder of Crows, after killing the Varterral and conversing with Zevran I was unable to loot the Varterrel. He had shinies I just couldn't seem to target him no matter what I did.
-Finding Nathaniel, as others have stated in the particular playthrough of DA:O I imported I killed the Architect, yet Nathaniel claimed that he had been spared.

I don't know if this was already mentioned, but I took the templars' side in the final quest and normally, Hawke should have become the viscount (is that the word, I'm not playing the english version). Yet no mention of him becoming it, and apparently no trophy (at least, I don't think so...it's one of the hidden trophies, right?).
Also (a small bug), I had a romance with Anders but chose to kill him for blowing up the Chantry. Well, at the end, Varric tells Cassandra that everyone ended up leaving Hawke except for Anders (I'm guessing because of the romance?), yet Anders was dead in my file....

I had a similar problem with the Fenris quest at first. I got mine to work by going to Lowtown and then entering the Hanged Man from their. I just dont think you can travel right to the Bar or it won't activate. See if this worksV_Burgh wrote...

Nathenial lived in Awakenings imported from Witch Hunt. Not sure it is part of the issue but I saved Amanarathine in Awakenings and left Nathenial at the Keep. I had all upgrades so he survived but the problem might be saving the town and leaving Nathenial at the keep. That can mean he dies if you did not fully upgrade the keep. Bug could be save Amanaranthine flag = Nate killed (if he was at the keep) even when he was not. Not sure if importing from Awakenings makes a difference. That would take a lot of game time to see. My current run through is with an Awakenings save where Nate was in my party at the end. I will see if the problem happens again.

When the battle against the crows ends, the dialogue with Zevram is the same of the first time you encountered him in the cave, than the mission doesn't end and Zevram is nowhere to be found.

I've heard (?) that Grace is supposed to kidnap whichever companion is being romanced if someone is being romanced. I was romancing Anders and ultimately got the achievement for it, but Bethany was kidnapped instead. (I also got the screwy "run off with Isabela") ending.
My first playthrough I was romancing Isabela and Varric was the one who was kidnapped. Then my second playthrough I was romancing Merill and Carver was kidnapped. So, I am not quite sure how they set it up.
