I had a problem with the Fenris questline in Act 2. When you go to the abandoned slave pens to find Hadriana, I forgot to take Fenris. There's no way out of the there except to go through. He participated in several dialog options (or, at least his ghost did), but wasn't there when Hadriana was defeated. Thus, the quest couldn't be completed. Even leaving the zone and coming back with him, there was no way to finish it. Not sure if that was a bug or by design, but it was frustrating nonetheless.

Wife had an issue with a certain pick axe quest for the Bone Pit. She tried to buy a pick axe from "smith" and the dialog just quits on her...

I just had this same problem right now cause I did the same thing you just did and came on here to find out anything can be done to fix it.  As I see, so far nothing
