 Hi, I have a bug starting in Act 2 a little after "Night horrors" quest (which has to do with the fade) 

Anders has the "Vengance mode active" even though I have not unlocked it nor activated it...

It's eating his mana, and he can't heal... It sucks... Would like to see t his patched. I've searched google to find that I'm not the only one with this bug.

After getting the second part of the "Repentance" quest and obtaining Sebastian as a party member, I go to leave the Chantry area and I can't. The normal trigger at the exit to "Gather the Party and Leave" does not show up. I'm trapped in the Chantry forever!

On a more serious note, it means I can't finish the Exiled Prince DLC because I can't get Sebastian out of the area...

I also encountered this. I found that if you've got the Panacaea ability for Anders, activating that mode cancels out the Vengeance mode.

Other issues I've encountered are ones mentioned several times already in this thread:
- Merril's initial quests (Mourning, etc.) skip straight to the Completed pile.
- After getting friendly with Isabela, her armour add-ons go a bit weird.  Nevermind, I'm just being stupid with this one...

I'm giving DA2 a rest until these things are sorted, in case any patch still needs me to go to before the issue occurs to fix.

- You cannot complete "Forbidden knowledge" when you read the first, second or third book and destroy the rest. You can complete it if you read the last one and destroy the rest though.
- "Sketchy on details" remains in your journal even if you handed it in and got the reward.

I also get the "Mourning" quest auto complete, and I made Carver into a Gray Warden and also did not get the "consoling words" that people were saying will come up instead if your sibling is saved.

Sorry if this have been mentioned but I didn't want to read all the pages.

I had once instance where, when wearing spiral robes, the staff would not show up while at the Wounded Coast. It was still equipped, just no object showing in the game world. Came back if I travelled elsewhere.

i have an act 2 bug at the moment. ive just killed the dog lords in thier alley. but the alley has no exits. my only save is from many hours before.(the auto save is inside the alley and thus same problem) i could not stand to actually repeat the stuff i have already done.

another bug i have is that if my main character (a duel wield rogue) dies in a big combat then gets reserected. if i then select an enemy to attack i run up. hit the enemy once and go idle again. comming out of combat entierly fixes this. but it can be very hard tap tap tapping those enemies just t make my go do thier proper job

Forgot to report this earlier (and I'm not sure if it has been mentioned) but when Dueling the Arishok using the "Decoy" ability from the Rogue Shadow Tree completely ruins his AI. After destroying the dummy the Arishok just stands in place and casts his Whirlwind attack every now and again. Stunning him breaks him out of his malaise and let's him behave normally once more (until another Decoy is summoned).

Hawke becomes completely immobile & unselectable in combat, it switches me to the first companion in the list and Hawke just stands there holding his sword. He is never attacked (apart from the occasinal accidental friendly damage but it does nothing to his health bar. Once he has become immobile in combat he stays that way outside combat. I got to a "gather your party" beacon and took out everyone and he still would not move. Only saving & reloading temporarily fixes this bug, until I enter combat again anyway...

Edit:  This has only occured in Act 2 so far, Black Powder Courtesy quest.

Final fight in Act 2 is bugged. I used Decoy rogue ability, and boss - after destroying the dummy - was just punching the air, like false Hawke was still there. It made this battle easy and cheap. Happens everytime when i reload my game.

I got stuck in the same place. I still had the second half of the Herbalists Task and was going to finish it (I still haven't found the tattoo ink, the guy said "no" and doesn't offer any other options.) Anyway, i also had Merrills quest for the mirror on Sundermount so when i entered the area the party select screen opened and it forced her into my party since she hadn't been there before. I did the quest with no problems and got back to the worldmap and figured i would do Fenris quest and it would force him into my party also. It didn't and once i entered the cave i could no longer exit. I did the same thing as above but was able to get out because it let me click *through* the door at the end (still closed) and go back to the world map. 
I went back to town and grabbed Fenris and went back to the cave hoping it would trigger, but the mage ladys corpse is just laying on the floor with the quest icon hanging over her and nothing happens.
I also did have he cutscene with the slave girl and invisibleFenris dialogue.

The barrels are not working for me at all. The only time they worked was
when the book bugged out and moved to a different location that I was
able to click and move the barrels. Now without the book bugged moving
to a different place. All I can do is click on the barrels and they
become highlighted and that is it..

The quest 'sketchy on the details' does not complete upon speaking with Sketch. Did recieve xp and reward. Journal entry has updated to 'You took care of the foreign assassins' but remains in incomplete quests section.

The "Sketchy on the Details" quest will not "officially" complete. Sketch gave me the quest reward, but the quest remains in the active (or pending) section of my journal instead of moving to the completed section of my journal.

The gang members for "The Lowdown" side quest are no longer spawning. I have completed all other quests in Act 2, with the exception of the final main quest, which I have not yet started.

To test the issue, I left the Docks (Night) level and returned again 20 times. I also tried a number of times after restarting Dragon Age II. Unfortunately, they still do not spawn.

Note that this occurred in a game in which the "Sketchy on the Details" quest is also bugged. I finished "Sketchy on the Details," received the quest reward and experience points, but the quest did not move from the active quests section to the completed quests section.

In Act 2 the “How to Frame a Templar” quest when completed causes the quest pointer for the boxes in the Gallows to not disappear.

 
In Act 2 the “Raiders on the Cliffs” quest you can keep talking to Lieutenant Harley for unlimited boosts to Aveline friendship meter. ( I am ashamed to admit I used this to max it)

 
In Act 2 the “Sketchy on the Details” quest when it is completed, doesn’t disappear from the journal also Sketch doesn’t disappear.

Side Quest where you have to search for the assassins all over Kirkwall which try to kill the elven mage doesn't complete. I talked to him after I killed them all and even got the Staff, but in the Journal it's still not completed.

Edit: Yeah Sketch's mission is what I meant ;)

Few issues - in the Wayward Son quest, if you talk to Samson (in Docks I believe) before the main NPC you're meant to speak with, Samson will remain tagged as your next stage of the quest for the rest of the game (it persists through act 3). The quest can be finished however. EDIT - my bad this actually belongs in Act 1 issues. Samson does however remain flagged through later acts.

The docks gang not spawning at all during act 2 is another issue - I've been switching between day and night, leaving Kirkwall, and moving around the area for quite a bit - they spawned only once and never again.

Not sure if this is an issue but at the end of the Night Terrors quest the Keeper gave me a book for saving the boy, saying it's very powerful. As it turned out it was simple junk of very little value?

Lastly - I have completed the Sketchy about Details quest, however the journal doesn't seem to think so and Sketch still has the quest marker on even though he doesn't want to talk to me anymore.

Is there a missing or broken quest in the Blooming Rose during Act II?

I went in there (just for a drink mind you!) and as soon as I walked into the main room I saw Trask talking to the madam. She says "Go on up, they're waiting for you." Trask walks to the back of the Rose, up the stairs and into the central room, which is unlocked. He then disappears without ever offering dialogue or anything like that.

Also, there is a mug at one of the back tables that is interactive, like a container, but clicking on it doesn't do anything.

Same here... no way to get back a save game from before, i did already about 5 quests after it because i forgot to bring fenris there first to check if it worked ...

Could use an idea on this ^^

My problem is this: once I finish big battles and the big boss is supposed to give you something cool, like in all the quests where you beat down gang members and the Blackpowder Courtesy elf, they don't glow! My map has little arrows over them, but I can't click them and get their cool stuff! Then, after a heartbeat, it says the quest has been completed, and I didn't get my stuff! I got it the first time, this just makes no sense.
