While we're making an attempt to capture the various issues posted in individual threads, this will give us a nice central place to begin collecting and addressing these.

These involve Merrill, I have seen a few reports of this very problem (and I have seen there are also problems with her in Act 2, fortunately it seems I avoided those.)
At the start of Act 3 immediately after the opening scene in the courtyard involving Orsino and Meredith.
I get a whole bunch of companion quests, but Merrill has two, "A talking to" and "The new place".
If I go to Merrills home first, Varric stands around in the doorway regardless of if he is in my party of not and Merrill is non responsive conversationally if in my party (or not there at all if I don't select her to be in my party).
If I do "The new place" at Hawkes mansion first, that completes. However If I then go to Merrills home I get a scene where she is in mourning over the death of the Dalish camp and a conversation option including "Whatever you do we will do it together" (or something like that). I believe this is the ending scene for her for Act 3 occurring early when I haven't even started that quest line.
At this point I can then talk to her again and "A talking to" completes, with Varric talking to her before the player character.
The Eluvian mirror then disappears from the corner of her bedroom. And it would appear I can go on my merry way with quests, although as this play through has her as my romance option I would rather it all played out correctly, and I am not sure how it will play out with this bug already causing problems.
Is there any way to fix this for ourselves? It's a shame to know the end of her story arc ahead of time but it happens I guess.

*spoiler* the fight with Orsanis the First Enchanter guy who transforms to the Harvester, in his 2nd phase the little guy can get jammed into a corner, Isabella can jump down the platform into the fog covered ground before and solo him, sometimes she kills it in about 5 min solo or not at all, i had to restart the fight a few times before Isabella was successfully able to solo it and the event resumed to phase 3 but she did not return to the platform, she didn't appear til phase 2 of Meredith's fight with the guard statues at the end.
also the quest "Who Needs Rescuing?" Orlanna does not appear in the camp to the north and on numerous restarts, the Bandit Leader just stands there unable to move or talk with him, he is in a neutral hue. Possible bug/incomplete quest?

Only issues I've seen so far aside from Fenris literally not having a questline at all. No clue if this is a bug from act 2 persisting or otherwise, however.
In addition to this, just ran into another one. I forgot to loot Javen at the end of Favor and Fault, not getting Aveline's final armor upgrade.
When I went back to collect it, though Javen's body was there and fully lootable, the armor upgrade was not.
