Act 3 Issues (may contain spoilers)

1. Quests finished, but not cleared from the Quest list:
- Act of Mercy (Rumor) - completed when it moved into an actual quest.
- Questioning Beliefs (Sebastion) - also done, but still on the list.
2. Quests on the list I haven't done, but not showing up on the map (no arrows and cannot just walk into the area to complete the quest):
- About 5 different quests...
As a matter of fact, I have only "On the loose" quest which has an arrow on the map, and none of the seven other quests still on my list...

The entrance is nowhere to be found. After my extensive search, I searched some more, this time narrowing my target zone to the area around Ander's clinic (as suggested by the strategy guide and various online guides). Nothing ever came about.

If I load my post-completion save, Hawke comes running in from the living room, her "inside clothes" covered in blood. The cutscene where Meredith shows off her lyrium sword triggers again, though instead of Meredith and Orsino the camera focuses on various bits of the wall. After it's over, the game appears to freeze; pressing Esc will end it, and things are normal thereafter.
Not a game-breaker or anything, but it's pretty hilarious

The area is exactly in the place of Amell Estate cellar entrance. Just press tab and it'll show a "secret lair" description. It isn't marked on the map but it's there.

-"Who needs rescuing" does not trigger (bandit leader is just a statue)
- Aveline's final upgrade does not drop
-"Red run streets" the hideout of "the Bleeder" does not appear anywhere.
-"Murder of Crows" I am not sure if this is a bug, but I cannot find how to start the quest. I am currently on "The last straw" quest, and I have done all possible quests, I believe, except for the bugged ones.
-There is an exclamation poit over my desk, but I check my desk, and there are no unread letters.

I got this exact same bug. The bandit leader is in the camp, but nothing and no one else is there. Entered and reentered the area several times, was always the same.

When visiting the Deep Roads the second time in Act 3 to look for Nathaniel, if your Sister was turned into a Grey Warden, you will meet her after the highway where you bumped into Temmerrin. After clearing away the horde of darkspawn assaulting her, an auto save is made right before the conversation with Bethany starts. Reloading using that auto save results in Bethany appearing in Lothering/ Act I clothing instead of the Grey Warden uniform she was originally in.
Edit:
In Act III, Olaf's Armoury appears to be missing his box sprites. Shows an empty store tent, but interact-able store. Not sure if it's isolated.

Oh, sorry, should have specified that too. All I could do was jump to the first plot point - meeting Velasco.

Templars/Mages chose to kidnap Isabela
I went to my Estate first before going to the coast
On my way out, they let me chose Isabela for my group
Isabela helped rescue Isabela
Stayed in my group until after the post fight cut scene (containing 2 Isabelas). Then after the scene she was gone from my group and only existed as the NPC standing in the camp.

Hawke is in a relationship with Merrill. At the beginning of Act 3, after going to visit most of the companions in their respective bases (and seeing Merrill in Hawke's Estate), I get a quest telling me to meet up with Merrill in her old home in Lowtown.
Upon entering, I'm greeted by a spoilerific cutscene that apparently shows the outcome of the quest I was supposed to do. I quit the game without saving at this point, and I've been told that this bug also makes it impossible to properly proceed with the quest.

In Act 2 Merrill destroys her Mirror, but in Act 3 she needs Hawkes help to repair it(Quest: a new path). There should be no mirror anymore.

REALLY a major failure from bioware here... One I usually do no expect in their games.
Imagine buying a BOOK and when you get to the last chapter you suddenly find the last pages in the beginning of the chapter. THAT would be bad. Then you find out that not only the book had the final pages first... But that the rest of the chapter has several pages missing?!
We would return to the bookstore to ask for a refund or a good copy.
Please bioware, do not make us want to return the book.
I was romancing Merrilll and for me that is one of the focus points of the game story, so the game is pretty much useless to me while you do not fix that.
Also I do not want to play and be afraid of other game breaking bugs happening, since it's clear that many quest triggers in act 3 are bugged.
Please, fix it.
You and Bethesda are the only companies that we rpg players buy games almost without asking questions.
Do not change that, please.

The thing about A New Path is that I can complete the quest but it never LETS me complete it really. It says its complete, but nothing happens as a result and the quest isnt really finished. I've tried playing it over again and again to see if I can get the right sequence or something in so that i can progress, but, so far, no luck.
Anyway, I've stopped playing the game until either I can figure out a way to get it to work, a community fix comes out, or Bioware themselves brings forward another patch.
Bioware, I love Dragon Age II. Even though it does have a few minor short-comings, its still a solid story driven experience, with great characters, interesting plot, challenging combat, and difficult decisions. These glitches are the only problem. Fix those and you've got yourselves a class A game here.

edit: Where is the official bug report forum for DA2? Because this isn't technical self help, this is bugs in the game that have to be addressed by the developers, not the players. <_<

Same, the bug still is not fixed. Could be like the fun Jowan one in DA:O. They never fixed that either, not in the 4 patches they released. Sad.
