A few spoilers maybe.
These involve Merrill, I have seen a few reports of this very problem (and I have seen there are also problems with her in Act 2, fortunately it seems I avoided those.)
At the start of Act 3 immediately after the opening scene in the courtyard involving Orsino and Meredith.
I get a whole bunch of companion quests, but Merrill has two, "A talking to" and "The new place".
If I go to Merrills home first, Varric stands around in the doorway regardless of if he is in my party of not and Merrill is non responsive conversationally if in my party (or not there at all if I don't select her to be in my party).
If I do "The new place" at Hawkes mansion first, that completes. However If I then go to Merrills home I get a scene where she is in mourning over the death of the Dalish camp and a conversation option including "Whatever you do we will do it together" (or something like that). I believe this is the ending scene for her for Act 3 occurring early when I haven't even started that quest line.
At this point I can then talk to her again and "A talking to" completes, with Varric talking to her before the player character.
The Eluvian mirror then disappears from the corner of her bedroom. And it would appear I can go on my merry way with quests, although as this play through has her as my romance option I would rather it all played out correctly, and I am not sure how it will play out with this bug already causing problems.
Is there any way to fix this for ourselves? It's a shame to know the end of her story arc ahead of time but it happens I guess.
that scene bugs the last part of her personal chain. You kill the dalish but you don't get a quest after that....soo that means the quest is bugged from the start.
Yea , it is kinda annoying
Also I have the ''Who needs rescuing ? '' bug too. Bandit Leader just stands there with no action whatsoever.
And also , I had the problem of Leliana's DA:O romance epilogue dialogue about when Hawke asks her '' are you The Leliana ? '' and she talks like she is just friends with the Warden. It maybe a small save issue about Witch Hunt ending or about Exiled Prince DLC ( since it is in Act 3 of the DLC ) but I hope we can get a fix about these Act 3 issues.
I didn't reach the last battle yet since I stopped after experiencing Merrill's quest bug. I hope we can see a fix soon.

I am also having issues with enemies triggering in an adjacent area before I get there (for example, when there are two walkways running parallel to one another, enemies are triggering on the neighboring walkway.
Edit: Met Gascard DuPuit running down the stairs away from me before I had even spoken to him in the 'Prime Suspect' quest. Reloading fixed the problem.
Edit 2: Sketch's quest will not complete. I found and killed all the assassins, found him, talked to him and got the reward, but there is still a quest arrow over his head and the quest is still listing as active. I cannot interact with him in any way.
Edit 3: Just finished the game. Awiergan scrolls 2 initially stated a hidden lair had been added to my map when it had not. Exiting and reloading the area solved the issue. As with many here, could not get the encounter for "Who Needs Rescuing" to trigger. The bandit leader was there but nothing happened. Aveline's final armor upgrade did not drop. Nathaniel was very much alive and well at the end of my import, but the "Finding Nathaniel" quest did not trigger. I got "Fool's Gold" instead and he was not there at the end. Enormous variation in the amount of damage done. The same kind of hit on the same kind of enemy would do anywhere from 30->400 points of damage. This included hits on flanked/stunned opponents. Towards the end of the game, special abilities stopped landing for any damage at all (archer rogue). Progressive slow down and graphic choppiness in the last 2 hours of the game. Had an exclaimation point over my desk and an envelope next to the mansion for most of the last act even though there were no unread messages. Was getting approval from Isabella, who was no longer even available as a party member let alone in the party, after confronting the Dalish clan during the last portion of Merill's final personal quest.

Also had Nathaniel use incorrect information regarding the Architect. My Awakening import game definitely had a dead Architect but he talked like it was still alive.
Hope these get resolved soon; I don't want to play forward due to broken quests.

Quick Question:
Is my dialogue bugged or does everyone involved in the quest Best Served Cold assume you're siding with Meredith no matter what you did before or how many times you tell them you're on their side....?<_<
I hope this is a bug. If it's not, it's a massive plothole. Though, the fact that the Mage/Templar rebels attack you on sight is itself a bit of a plothole. They all kind of assume I'd side with Meredith based on absolutely nothing at all. It doesn't really make sense. Compounded by the fact that Thresk (Sp?) says what he says about The Champion siding with Merideth, when I clearely did not; I can only hope it's a bug.

At the end of Act 2 I had really high friendship with Fenris and had completed the romance plotline thus far. When I started Act 3 I completed his quest "Alone", persuaded him not to kill his sister, and then the loading screen took us all outside. After that there were no further quests or conversations with him even though he had the little Hawke coat of arms on his waist. I thought maybe he was sulking and the conversation/quest would pop up later. However, before I knew it I was up to the final quest (100% friendship, gave him all his gifts) - I chose the mages and instead of having any option to persuade him to help, he just stormed off with his "I'm not sticking around to help mages" line.
Having read around I now see that I missed out on the rest of his questline (i.e. apologising for being a pantywaist in Act 2 and being asked to move in) which was meant to trigger after "Alone" was completed. I reloaded my last save game prior to Act 3 (half way through act 2!) and replayed all the way through it up to that point... only to find that it's still not working, regardless of if he kills his sister or not. There's no follow up conversation or anything.
So, having wasted a huge amount of time on this bug, I'd really appreciate it if someone could fix it.

I am unable to kill the Gate Guardian during the Battle with Maredith. She summons the Guardian after a quick fight and when I am in battle with it, am unable to kill it. It remains at 1% and I cannot progress. Ive tried starting over from different saves and am using no mods.
Any quick reply or information regarding any soulutions would be appreciated.
same thing for me ..will not die and can not finish the game.

Same here, he didn't leave the party after completeing the quest, there just a loading screen and everyone was outside the hangedman......then nothing, no dioluge, no quest. He didn't show up ask for my Hawke's forgiveness or any other thing the guide book mention.

The quest "Who Needs Rescuing?" on the Wounded Coast. In the same location as the Flint Company mercenaries from the Act One quest "Duty" there's a man just sort of standing around. Holding tab doesn't highlight him, but if you hover your mouse over his model "Bandit Leader" pops up as his name.
Having the same problem with this. Most hospitable bandit leader i've come across.
