As far as I know I have completed the romance
with Isabela and the achievement has not popped for me, so I figured I'd
ask here and see if there is something I have yet to do.
Some background on where I'm at:
I am in Act 3, though not far in. Mostly been doing side quests, haven't advanced the main quest past the first argument between Meredith and Orsin.
With
Isabela I have completed all her companion quests, given her both
gifts, have her maxed out as a friend, and had the conversation with her
in the Hanged Man where she's all "Thanks for waiting for me, blah blah
blah" then she and Hawke kiss.
After this scene concluded I
figured that'd give me my achievement, but it did not, so I thought
there may be some way to ask her to move in as I had heard that's
possible with romances in this game, but I have not found any way to do
that.
Without giving me TOO many (some are okay so long as they are necessary to get the achievement/see the romance play out) spoilers
is there something else I need to do with her to complete this? When
can I do it? Does it come naturally, or will I have to find a specific
way of doing it?

Out of a morbid sense of curiousity... why are there screen shots of a semi clothed Isabella love scene with hawke but in game they are clothed? The screen shots came out just before the demo so I don't think anyone made a mod that quickly.

I seem to remember a few people saying they didn't want the underwear sex scenes. I'm not sure if they were hoping for more or less. But they got less.

Yeah. I didn't romance Isabela, but to my knowledge, "completing" the romance means staying romantic all the way to the very end. Just make sure you talk to her again before the finale (you'll know when).

I got it early in act 2 I had only talked to Isabella once at the hanged man without doing any of your companion quests yet. I just selected the the Heart option in my first conversation with her then I went back to my estate and it triggered the love scene plus achievement. I also romanced Merril right after her and she was the one I asked to move in with me which got me her new armor. Isabella then asks about me and Merrill afterwards in the game threatening to cut my balls off if I hurt Merril.

You get the achievement, after you pick your side near the end. She'll say something about not wanting to lose you and you can comfort her or something, then Hawke and Isabela kiss and you get it.

This. The achievement pops right before the final battles begin.

Tach, sure you're not thinking of the like "Flirtatious" achievement? (I can't remember off the top of my head if that's exactly what it's called, but it's close to that.)

I missed that as well. I did get the ending though where Hawke and Isabela stayed together, so i don't care, i'll get the achievement in my next playthrough.

In my guide, it says you must also give Isabela both of her gifts to get the full completed romance, so that could be why. But like the others said, you must stick it through till the end and talk to her just before you fight against the mages/templars.

Rivani Talisman, it's in the Disused Passages (accessed via Docks at Night) in Act 3.

Hey I never said it was right, it's just what it said. *shrugs*, it was the official one but it could have made a mistake, I was just putting in some input to help out the OP.

And I'm not blaming you for posting the guide information. I'm merely saying that the information is incorrect so that if something goes wrong people don't start thinking that's the reason why.

I just got the I love you hawk scene for the questioning beliefs part but now it says when I give her the second gift the only flirt part is that I'm not looking for love? What is the correct romance response?

I don't believe it matters. It makes more sense to view that scene before the final one, because Isabela's all, "Hawke. I'm not looking for love... or AM I?" but it doesn't change if you watch it after completing her quest and Questioning Beliefs. Don't worry about it.
