In DA:O I killed Flemeth so in DA2 when she comes back out of the Amulet she has the same body and look. Correct me if i'm wrong but doesn't she need to find a new host every time she dies?

Seems not, I also "killed" her in DA:O.
She clearly hints that this ceremony whith the amulet, was a method of surviving morrigan attack on her, that she predicted, Flemeth reveals you have just saved her, has she had saved you, she also said that morrigan wasnt either her daughter or enemy.
Then she transforms into a dragon and away she goes, the end.
