Ander's quest "Questioning Beliefs" sat in my journal, and Ander's even had a quest mark above his head, but I couldn't talk to him at his clinic. Just his usual remarks.

This is the quest, if memory serves me correctly, you get in the same line of quests you help him by distracting the Grand Cleric so he can, if I understood it right, plant a giant lazer bomb that blows up the Chantry.

I didn't seem to have missed anything from the quest but possibly a simple conversation, but still it urked me. Did this happen to anyone else?

Even without metagaming knowledge I knew he was up to something shady, him not disclosing any details also didn't help.

This just happened to me on this playthrough. I don't remember it on my first, though.
