I've looked and I haven't seen this mentioned anywhere.

When I first turn on my computer then start DA2 the graphics are fine.  Then, if I exit the game and then go back into it, parts of my characters, maintly the player character will be missing.  Face, legs, sometimes the whole body and it's just her weapons floating where she should be.  Sometimes she's gone entirely.  VERY occasionaly the sisters face will dissapear or be all black.

If I completely restart my computer everytihng is back to normal.

I also have the problem with the no disc error popping up when I start DA2 but hitting cancel till the message goes away gets the game started.

No idea if I'm listing this information correctly but here it is.

8GB physical memory
ATI Radeon HD 4200 Vram 256
Windows 7
DirectX 11

The no disk error should be fixed as of the Patch 1.01 BETA - http://social.biowar...0/index/6482838

The other issues seem related to your graphics drivers. See the DirectX 11 thread for more info on how to update your drivers for DA2 - http://social.biowar...0/index/6420212
