If you don't talk to her before leaving Lothering you can't go back and she's dead.

Does the Codex state this, or is it not updated? She could've escaped somehow.

The only way she can 'die' is during the sacred ashes quest. (if you do bad, bad things.) You get a chance to mention that when you meet her in DA2 and she says the Maker decided it wasn't her time yet.

I swear nobody complained this much about Oghren, and you can murder knife him in camp! No excuses! Strip the carcass and burn the body!

.... then again if I had done that, the whole camp would've likely exploded.
