I lost Isabela after Act II, she never showed up.

I supported the mages immediately, and Fenris switched sides, but I eventually managed to convince him back to my side.

And I killed Anders. I never liked him in Awakening anyways.

So, one disappearance, one death. Out of a party of eight, six survived.

I killed Andres, Bethany was in the Circle ( went templars) and Isabella left with the book dispite me promising her it.

Isabela took off, Sebastian took off, I sent Fenris off after his intro quest, and that was really about it.

Oddly enough, I could still tamper with Isabela's equipment after she had taken off...

I lost two people. I gave Isabela to the Qunari and killed Anders.

Lol I was the exact same as you, OP.

I thought with the odds against us, I'd lose my party members whose trust I hadn't really earned (such as Fenris and Sebastion) in that final battle, but everyone came out alive, perfectly sound. I guess ME2's Suicide Mission spoiled me. The stakes were so high going into that grand finale, but I didn't feel like that finale lived up to everything else.

For me, the Chantry exploding was the climax. Meredith was just an encore to give us a challenging boss battle. Why the hell did she even have the idol? It ruined the character for me. She used to be respectable in her opinion despite me not agreeing with her, but now suddenly she's been brainwashed by power this whole time? Lame.

Carver decided that joining the templars was a good idea even though he said and I quote "I've never met a templar who isn't a ******". Isabella ran off because I didn't bother using her at all during the game so I couldn't raise her friendship.

Only lost Anders, which I think is okay. Really liked him in Awakening, but in DA II he was just too crazy. Still not a decision I made happily.

Surprisingly Aveline stayed event though she first was pretty much unsure if my Hawke's siding with the Mages was a good thing.

'bella bolted with the book, Bethany joined the Circle, I shivved Anders and I unfortunately had to stab Fenris in the face. (Is there ANY way around that if you don't have him rival'd/friend'ed?)

I didn't lose anyone. Isabela came back and Fenris took barely any convincing at all to get back.
"ARAWRGH I can't support you in this!"
"Dude..."
"Oh fine, I'm coming."

I guess it could be argued that Bethany is lost no matter what happens to her but she was there in the end at least, so hey.

I lost the elf warrior who joined the Templars. Isabella came back for some more sexy time after she stole the book. Bethany came back near endgame, I let Anders live since he was my main mage damage.

So only 1 here.

I guess you count Bethany, I never actually recruited Fenris, i didn't realize the quest was to recruit a character, and Aveline was getting a bit antsy about the whole thing so I dropped it. I did think it was odd that there was only one Warrior (Although the little bit I've played with him since I find him incredibly annoying). I lost Vael cus I wouldn't kill Anders, I told Anders to get lost and we weren't close enough for him to come back, and Isabella left before I fought the Arishok. So I ended up with 4, also I lol'd at Hawke telling Varric to "Stay out of sight and use your crossbow", yeah great tactical advice there pal, I'm sure he wouldn't have known to do that on his own.

Lost Isabela in Act II, Carver joined the Wardens but he came back to help me in the end (I squeed so hard, I love Carver!), then I killed Anders.

I got Fenris to full rival so he stayed with me even though I sided with the mages.

Isabella took off with the book, never came back. Sebastian left to raise an army against Kirkwall/Anders & my Rogue M!Hawke. And Fenris got multiple stab wounds due to not wanting to side with me, even though I asked him nicely.

 everyone (including Sebastian) except Varric, Merrill, and Aveline left by the end of my first playthrough, although, both Bethany and Anders returned for the final battle. First, Carver died, than Bethany got taken away by the circle, than Isabela left when I told her the current situation took priority over her relic, than Sebastian left when I refused to kill Anders, than Anders left when I told him to, and then Fenris left when I sided with the mages at the end.

Isabella ran off, Carver was inducted into the Grey Wardens, Anders was shanked, and Fenris was never recruited. (His sidequest was the last quest I had to do in Act 1, and me not knowing that I recruited him in that mission decided to skip it since I did pretty much every other sidequest and was tired of doing them.)

I lost Fenris because I never used him and sided with the mages, and Paul Ateides for not killing Anders.

Carver at the start of the game, Sebastian left, because I didn't kill Anders, everyone else stayed.

 I lost Anders when I killed him after siding with the Templars, but then I redid the end and sided with the mages, sparing Anders but losing Sebastian. I had 100% rivalry romance with Fenris and I expected him to possibly leave and rejoin me at the Gallows (because I'd heard of that happening), but all he did was say something like "I don't like this but I won't abandon you now." Hawke didn't even have to argue with him. :wub: It was sweet. Although I have to admit I think it might been interesting if he had left me at that moment only to come back out of love in the Gallows. Maybe he only does that if you do a friendly romance...?

Anyways, I never lost anyone else. Carver was the only one I didn't reach 100% friend or rival, but even he came back from the Wardens to support my mage Hawke (in both my Templar and Mage endings).

Isabela took off, and after Sebastian threatened to kill Anders, I told him to go to hell and become the prince he's meant to be, and not some boy running away to the Chantry.

Or...was that last bit all in my head? Yeah. Sebastian just left after I refused to kill Anders. :D
