I've only lost Anders at the end of the game, I'm a major good guy, so I just told him to get lost,Isabela returned before fighting the Arishok since I was in a romance with her.
How many people did you lose before the end?

Whoooa how do you not lose Carver? I thought his death was scripted. I killed Anders because he was a very awkward character. I got the feeling that he was coming onto me. He is the first RPG character that I have purposely killed off. This is saying something because I always play as the good guy.

Anders because I had to kill Anders... I had almost full friendship with Sebastien and I couldn't look him in the eye (...proverbially) when he demanded to know if I would be waffling if he had been in there. He was right. He could have been in there. We don't know WHO was actually in there. Peasants had no reason not to be praying in there. We only see a section of the Chantry when it goes. And the most corrupt of the Chantry, Petrice, was already dead by arrow and rightfully so. Instead, what we do know is the grand Cleric, who wanted to find a way for both sides to compromise and even said she would die for Kirkwall if she had to.. had to die to the hands of a mage that just wanted to start a revolution which would had ultimately totally destroyed Kirkwall if our Champion hadn't picked a side and fought to end it earlier. You saw the destruction in the streets.
Isabella because I just... didn't care about her. And Bethany was with the Grey Wardens but I didn't really consider her 'gone', She understood my decision to side with the Templars, saying if I believed it was the right way, it was. I was all nervous when I saw her again and Varric muttered, "She's not going to take this well..." I wanted to reply, "...I know. Crap. How do I explain to her all of the blood mages I've seen this last year?!"

I lost Isabella and Sebastian. I kinda mad about losing Sebastian because I quite liked the guy, and Anders deserved to be exiled at least for what he did. I needed a freakin healer though, so gameplay dictated that I had to keep him

Isabella ditched me and never came back.
Sebastian got POd because I had to much attachment left for Anders to kill him.
Fenris left, but came back.
Carver never came back.
Ended with me, romanced Merril, Varric, Aveline, Anders, Fenris and my dog, Garrus.

Ah yes, I forgot about Sebastian.
He left too.
He shan't be missed.

Isabella left me and never returned.
I ended up killing Anders because I was worried about a future war. (Though to be honest I doubt Sebastian would be able to make good on his threats)
And that's about it.
I plan on taking my siblings to the deep roads with Anders in future play throughs. I'm hoping to avoid any difficult decisions in Act III or having any family members being used as pawns by the mages.

Isabela came back, and I wasn't about to let the Arishok take her. Anders had to die, being all kinds of crazy, and Fenris turned on me, in spite of calling me friend.

Uhhh...
On my first playthrough which I finished earlier:
-Never got Isabella (Didn't know at the time, thought she was in Act 2)
-Killed Anders
-Convinced Fenris to come back to me even though I never used him.
Carver joined the templars after Act 1 so never used him.
Mainly used Myself (primal mage), Anders (Healing), Whats-her-name guard captain, and then Varric. After I killed Anders I used Merill who i liked but has far too little HP for my liking and seemed to run out of mana and hp in the blink of an eye

Didn't lose anyone. Was friends/lovers with Isabella, and I dueled the Arishok to keep her. Be sure to talk to her in the hanged man at the start of act 3 and she'll rejoin. I sided with the mages (didn't have the sebastian DLC), and fenris decided to leave the party. But i asked him politely to come back, and he did (NBD, i hardly ever used him) Thought I was going to lose aveline too, but she stuck by me. Even got bethany back for the last sequence, so i got to lay the hurt with elements.

By the end, I had 3 leave my party for good:
Bethany died in the Deep Roads; Sebastian left after I sided with Anders (I didn't want him to leave, but I needed a healer and I could easily replace Sebastian with Varric, lol); and Fenris was dead after he sided with the templars and tried to fight.

I lost Isabela and Fenris betrayed me. But I'd romanced Anders and couldn't bring myself to kill him, so instead Sebastian vowed vengeance and stormed off. With the city in shambles. I get that you're upset, but really? I'd understand fighting on the other side like Fenris, who'd been my rival anyway. But to just leave... *sigh* Oh well. Now that I'm a bit more familiar with everyone's personalities, I'll be trying to remedy this. I still won't keep Sebastian, though.

I only lost Sebastian because I would not kill Anders. Everyone else was at 100% friendship... and that took some doing, let me tell you. Except for Carver. I had a hard time figuring him out, so I wound up in the middle with him, neither friend nor rival, but he stayed with me anyway.

Sided with the Templar, so I only lost Anders .. it seems like if you side with the Circle, you will lost more companions.

Bethany ended up in the Circle (even if I got her back at the end), Anders ate the murder knife, and Fenris died fighting alongside the templars when I went mage ending.

I too, lost Isabella that greedy little jackass. Can't believe she would take the book. I mean, has she not seen how much firepower Hawke's group packs? They would totally be able to protect her. Anyway, to hell with her.
Sebastion also left because I wouldn't kill Anders. Partly because I needed him as healer and also because I think that having him do good from that point on would be better than ending his life and losing a powerful healer.

Bethany joined the circle but came back towards the end, Isabella ran out on me and i killed anders. Also i didnt know fenris could leave you if you sided with the mages. i guess he stayed with me after i sided with the mages because i maxed out his rivalry.

Isabela ran away after inciting the Qunari to war.
Fenris sided against me and received an axe to the face.
