So if you do well with your companions, you will only lose Anders(kill him or side with the templar) or Sebastian(not kill Anders or side with the circle).

Not necessarily. Merrill will turn on you if without a full friendship bar.

Only lost Sebastian because I wouldn't kill Anders. Everyone else was at 100% friendship and stayed with me.

I executed Anders for being the low life terrorist that he was (damn game did not let me kill that abomination earlier).
I killed Fenris for turning on me at the end and wiped the goop that was all that was left of him from my feet on Merideths burning corpse.
Everyone else was with me and survived.

Carver left for the templars, although he returned in the end. I lost Isabela just when I started liking her (I started liking her the moment she said "I AM GOING TO DIE" just to get my attention from Aveline. Right then and there I loved her!) and now I want to beat her with a rock for betraying me 
I also temporarily lost Anders due to a bug, but I redid that part and got to keep him, yays!
Also Fenris turned on me, but I got him back, too.
So really, in the end, I only lost Isabela. I wish there was a quest to go after her! :/

Bethany became a Grey Warden, and returned for the final battle.
Isabela brought the artifact back and I fought the Arishok in order to keep her.
I sided with the mages, and then went one further and sided with Anders (I was romancing him, as well as playing a "the ends justify the means" character - we got on together beautifully =P).
Killed Fenris (he was an ass, anyways - I was happy to do it).
And, finally, Sebastian left 'cause of the whole Anders thing (he was an ass, too).
All in all, I'm quite pleased with my final party members. I got rid of the two characters I didn't like, and in the end I got to run off into the sunset with my favourite crazy apostate. =P

I didn't lose anyone but Carver lol, yeah Beth left with the circle for the main part of the game, but since I don't have the DLC for the prince. I joined the mages (because the Templar's are nothing but compensating extremists) and sent Anders away at the end, but he rejoined before the battle against the first enchanter, and Fenris rejoined after I told him he was supporting slavery. So I feel pretty good about the party count at the end. I just wish that it had a little more after the ending, felt a little irked when I went to finish a sub quest and the end of the game started because the sub quest was at the Templar area.

Carver ditched me for the templars, bastard. Isabela came back after i gave her the book (and we happily fell in love at the end
). I let anders go but he wanted to die anyways at the end, so i killed him. Everyone else stayed alive and fought with me

Lost no one, spared Anders after sitting for nearly 10 minutes trying to decide, I really wanted to kill him for what he did but was unaware Bethany would return so figured Anders was my only really decent healer option hence spared him. Isabella left then returned with the relic, and Fenris sided with templars but I nicely asked him to return so he did. Was romanced to Merrill and natrualy sided with the Mages.
Next play through that terrorist swine gets beheaded for sure. Might romance Isabella instead also since the epiologue verric said everyone left me with time bar Isabella.

Carver died in the Deep Roads.
That was the only one. Fenris joined me at the end - which was weird considering his approval bar was about halfway. I almost killed Anders, but held back in the end.

I lost Sebastien (who left after I spared Anders) and Fenris. I'm guessing you have to have full rivalry with him to get him rejoin you if you side with the Mages. I was very close to that (though I did notice that I didn't get any achievement credits for that and a few other things that I did).

I was expecting to lose Aveline, Fenris, and Sebastian but only lost Sebastian at the end for not killing Anders. I was on the friendship path for both but neither was maxed out....guess it was enough.
