Killed Fenris at the Gallows, nothing of value was lost. Sebastian left, he was okay seemed like a nice enough guy. But Anders is my slightly distant but still best friend, friend. Besides I like his "I just want to watch the world burn" demeanor at the end.

Isabella left in Act II and I was fine with seeing her go. Sebastian left when I refused to kill Anders. Fenris left at first when I sided with the mages but came back soon after. So I lost... 2. Honestly would have been happier if I'd been allowed to shank Seb and Bella.

That was it. Sided with templars, and Merrill had a full friendship bar so she stayed with me.

Sebastian left when I wouldn't kill Anders. Sided with the mages (with Carver as a templar) but he helped me out in the end so it was all good.

Lost Isabela in Act 2 for obvious reasons. Temporarily lost Anders after he blew up the Chantry, but made up with him just before the final battle. Lost Fenris for siding with the mages, fought and killed him. Lost Sebastian because I spared Anders.

"Lost" Carver to the templars, killed Anders, killed Fenris and Isabella never came back. So I lost about half my entourage at the end.

Isabella ran off, my sis went to the Circle and that's about it. Also Fenris went to Templars when I sided with msges, but i convinced him to comeback even though he was not a friend or rival (Used him only once or twice). Don't have Sebastian but from what I'm reading letting Anders live would have me lose him. Also in the last Varric narrated scene it said everyone eventually left... Except Isabella of course? What's that about, is she following me or is it a messed up flag, i romanced Merril but I did flirt with Isabella.

First play though as male Warrior.
First Bethany Joins the Circle.
Then Isabela left with the book and never came back(tho, I thought I built enough of a friendship, guess not).
Anders went Crazy so I knifed him.
Bethany Returned when I sided with the Mages.
Fenris siding with Templars, I'm with the Mages. I didn't even bother to talk him back, just killed him.
So three. Isabela, Anders and Fenris.

I too had this flag problem, my male warrior romanced Merrill but Varric claimed it was Isabela who followed me.  My warrior did flirt but ended any romance.

Isabella never returned after she took off with the relic, which is good since I would have killed her if she had.

And I lost Anders, to the murder-knife *sniffle*

And then Sebastian makes a pass at my character 5 minuets later at the Gallows.....Horn dog hiding behind the chantry.

Killed Anders for the psychotic terrorist that he was.

That left me with Merrill (), Aveline, Sebastian, Varric (we're bros) and Fenris.

i lost none and i didnt do the exiled prince yet. so he dont count next playthrough i will so in advance i lost one.. lol

I supported the mages immediately, and Fenris switched sides, but I eventually managed to convince him back to my side.

And I killed Anders. I never liked him in Awakening anyways.

So, one disappearance, one death. Out of a party of eight, six survived.

Carver got his insides crushed by an Ogre, unavoidably so we won't count it.
Bethany succumbed to the darkspawn blight in the deep roads.
Fenris left my company when I sided with a seemingly reasonable Orsinis, and during our later confrontation I finished what I started. 
 
2 Deaths. Considerably less well than my first playthrough of ME2, where keeping the entire party alive was absolute child's play.

Lost Carver at the start of the game (same thing happened when I started a new playthrough so I'm not sure what factors into this), Isabela disappeared with the relic, Sebastian left because I wouldn't kill Anders, and Fenris turned on me because I sided with the mages so I had to cut him down - which made me sad because he was a constant in my party until that point. :<

I was half-expecting Aveline to leave as well but surprisingly she didn't. Does this change if you don't max out her Friendship/Rivalry?

Well, I lost carver in the deep roads,even though he was a douche, I felt it when he died...
Isabella left but came back just before the duel with the qunari commander.
I let that Anders go, Damn him, but he sided with the mages so i killed him.
I romanced merril so she stayed with me after i sided with the templars, I think it was the best choice though I played as a mage.

Bethany became a Grey Warden, Sebastian left like a b****, Isabela left, and Fenris betrayed me after siding with Mages.

I never liked Sebastian.

Not long after killing the Arashok the only party members I've lost so far are Carver (to the Ogre at the start) and Bethany (to the Drakspawn corruption in the Deep Roads). I've romanced Isabella and she's stuck around but I haven't seen Sebastian yet.

Edit: Ah! That'd be why I haven't seen Sebastian, he's the Exiled Prince. I haven't bought any DLC yet. I wanted to check the game out first. I might get him later although you really don't need any more companions than what is already there.

Carver went Templar, Bella left with my book, and Sebastian left when I kept Anders alive. Didn't really matter, needed a healer more than another archer. Plus I was playing a mage so siding with mages goes without saying.
