Bethany is a companion if you go Warrior or Rogue, Carver is who you get if you go mage.  Depending on certain choices you either lose them after the deeproads, or they are gone till the end fights.

I had everyone at the end except Anders.  After he suckered me by blowing up the chantry when i was trying to keep the peace, he just had to die.  I had Isabela at good enough friend level that she came back after the incident with the qunari and stayed around for the last act, even though I had given her Castillon's ship and was romancing Merrill.  I sided with the mages and Fenris originally joined the Templars, but even though I only used Fenris on his quests apparently it was enough to get him to switch back to loyal.  Sebastian was the same in that I didn't use him most missions ( since I already had Varric as the archer support) but he stayed loyal to me the entire time, even though i sided with the mages.

Isabela ran off and never came back. Sebastian left when I wouldn't kill Anders. Fenris left and came back pretty easily.

Carver went to the Grey Wardens in the Deep Roads, but made an EPIC return for the final battle. I was soooo pumped to get him back. Favorite companion - easily...

I didn't kill Anders because he was my healer. I didn't want anything to do with his plotting in the first place, but I'm not sure how I would have gotten through the game without him.

The first play through, I was just making decisions on instinct. Next time through, I'll try to keep people around. I'll be playing a warrior next time, so I'll miss Carver .

Lost Bethany to the Grey Wardens, she came back. (If she joins the Circle does she still come back?)
Lost Isabela to the book but she came back and I kept her.
Lost Anders due to Hawke killing him, but changed my mind and reloaded and let him live.
Lost the Exiled Prince because I didn't kill Anders.
Lost Fenris for supporting the mages, got him back.

First playthrough I was blind.  I missed getting Fenris, (so technically I didn't 'lose' him but never actually had him).  I murderknifed Anders, Isabella disappeared with the book (and never came back since I missed getting her friendship up before the final quest).
For the final fights I had Merrill (as auxillary) and in my group, Aveline (tank), Varric and Sebastian as ranged DPS and my LadyHawke SH Mage.
so...three down, 4 stayed.

Didn't kill Anders, really wanted to but I was romancing him and I needed him to live to complete my romance achievement...I'm not gonna be so kind in my next playthrough. everyone else stayed...even Merrill...guess 100% Rival worked somehow.

I gave Fenris back to his master, Carver to the templars but I think he sided with me in the end(dislike the no conversations after the Meredith fight) and the DLC bow guy who I never used told me he was going to bring down the wrath of some city on me.

Fenris left briefly, but rejoined me.

I just lost Fenris. I didn't really like him anyway. Maybe I should reload and try and talk him back.

I didn't even get Fenris in my first playthrough, i think it was because i told the quest npc to bugger off. Bethany was in the circle, and Isabella took the book and never came back.

I didn't kill Anders, I was romancing him but I did break up with him

Emelie Hawke killed her love interest Anders, and Fenris died in battle opposing Emelie as she fought for the mages.

Everyone else fought by her side and survived, including her sister Bethany who had lived inside the circle for several years.

On my first playthrough, I killed Fenris because he didn't like me siding with the mages and all that.
Screw convincing him to come with me, I decidedly assassinated and one shotted him.

Sebastian left, but that isn't really that bad a thing.

isabella came back and i fought for her honor or w/e.
fenris flipped sides but came back after i was like "come on dude"
anders i told to gtfo and never come back, but then he came back lol.
carver was a warden and then came back and was like "sup! brother hawkes ftw! *brofist*" and totally redeemed himself for being such a douche in act 1.
married merril (well it never said so, but they were living together for like 5 years? that counts in my book lol)

I guess it could be argued that Bethany is lost no matter what happens to her but she was there in the end at least, so hey.

This made me laugh and it sums up what happened in my game as well.

Only Carver ( at the beginning ofc. ) and Sebastian. Sebastian - because my Hawke refused to kill Anders. Fenris was my rival but even he fought at my side - Best thing is that we fought against the Templars.

Had to kill Fenris; he refused to join my mage cause, despite being at 100% rival (actually the game seems to be bugged for me; I didn't even get the rival achievement). Sebastian stormed off because I wouldn't kill my boyfriend. Carver left to join the templars but joined my side in the final fight. Everybody else stayed with me

I killed Fenris after he sided with the templars, told Anders to get out of my face and Sebastian left because of the decision with Anders.

Bah, who needs these guys anyway? Never used them. All my other trusty friends fought with me. And Isabela even stayed with me in the end.

On my first playthrough...killed Anders, killed Fenris, Beth/Carver/Mother all died, Isabella left no return...at the end it stood just Mage Hawke (M), Aveline, Sebastion, Varric, Merrill (Romance)
