I believe siding with the Templars nets you it at the end. I'm guessing you sided with the Circle?

At the very end, choose to help the Templars.
That's all there was to it for me, as far as I could tell. There were some dialogs along the way where I said I wanted to be Viscount and was basically told that I had to have Templar support, and there was one dialog where I let some surrendering mages go free because Cullen wanted me to, and in the end I think he is the one that approves of you being the Viscount.
