How do I make Isabella return the book?

Her friendship meter was pretty high at the time though.

IIf you choose to give the book to the Arishok, Isabela will return in time for the duel to return it IF you romanced her. AFAIK that's the only way to make her return to your party. You can also choose to let her give the book to Castillon, which eliminates the possibility of resolving the conflict peacefully (doesn't really matter, as you have to kill the Arishok either way).

Doesn't matter whether he respects you; if you want to keep Isabela in your party (and alive, for that matter), you have to fight and kill him. The Qunari will leave after that. Say what you want, but these guys stick to their deals.
Also I didn't always agree with the Arishok, yet he said that I "command respect".

Question, if he doesn't respect you, can you still duel him?

I don't think I got his respect in my game, but I had to duel him anyway for Isabela(which I couldn't do as a spirit healer mage sadly)
No idea what happens if she doesn't ever come back.

You can gain his respect in the prior quest when you meet him at his compound with Aveline.

If you have a high enough friendship or rivalry, or if she was romanced, she'll return (blaming it on Hawke's influence). It's at this point you can duel the Arishok for her life, or hand her own to the Qunari.

I said he shouldn't keep the elves.
I had max friendship with Isabela too, and I had never romanced her.
