I had less trouble dueling him with my Spirit Healer mage (he couldn't kill her in one blow and her aura kept her regenning health as soon as it dropped). She kited a lot and fired her staff at a distance because its hard to fight when you are picking yourself up off the floor, but my warrior still had to kite him around and peck at him when he did the charge into the wall thing. Don't hide the evidence of burned bodies, talk to him several times during the main quests... he will disagree with you a lot, but he can respect you for your honor. I was pretty surprised when that happened for my smart-mouth snarky mage.
And yes, it doesn't really matter in the end. But its a neat addition.
Question, if he doesn't respect you, can you still duel him?
I don't think I got his respect in my game, but I had to duel him anyway for Isabela(which I couldn't do as a spirit healer mage sadly)
No idea what happens if she doesn't ever come back.
I think the approval has to be jsut about 50 or so (to get her questioning beliefs quest in Act 2). When the time came I told her she could have the book (hey Hawke didn't know the Qunari were gonna go on a rampage). In the end she came back to Hawke complaining that I'd corrupted her lol.
