How exactly do I gain the Arishok's trust? My guidebook refers me to a page that says nothing about it, so can someone help me?

When Aveline asks for the elven fugitives, agree with the Arishok rather than her. He'll have the qunari attack you, and you'll get the achievement.

I think it is also tied to whether or not you advise to burn the bodies of the murdered qunari or tell him the truth.
In my playthrough I got that achivement by telling him the truth, I think even prior to the elven fugitives quest.

I also don't think that after discovering the bodies you HAVE to go to him... but if you go you'll have a little conversation with him.

The way I got it was after getting the quest "Offered and Lost", I went to the Arishok and told him about it. At the end of the quest, tell the viscount not to hide the incident. Finally, after that but before the next Qun quest starts go to Arishok and let him know who was behind the incident. That's how I got the achievement.

1. Tell him about the delegates missing.
2. Don't hide the torture of the bodies. (I think admitting you killed them can also work)
3. Agree that the elves did the right thing when killing the guards.

In general, when dealing with him be honest.

You don't need max respect to get the duel.

The first instance is after the Shepherding Wolves quest (only if you fought the Qunari), if you go back and talk to the Arishok and tell him flat you killed some of his man, you will gain respect for the first time. I gained the achievement right after the Offered and Lost quest, siding with the elves in Demand of the Qun was not necessary.

There's several points where you can talk to the Arishok, including some that might not be marked with a quest I think. Agree with him every time and it'll pop. I got it my second playthrough before I had to fight swarms of Qunari.

Actually I didn't even get the option to tell him.. Very odd.

All the Gaining Honor thing should happen in Act 2, and not 1.

The one case I missed is I told the Vicount to hide the Qunari dead bodies (beacuse of marks of torture)  that made me lost the point I need to gain the achievement.  Eventhough he still respected me enough to say so and offered single combat (but I think the Duel thing is if Isabella came back and not because of respect).

It seemed pretty obvious from a role-playing aspect that you should be straight up with the guy.

Hmm, I got it before the elves even came up, right after telling him about the torture of the delegates. Good rule is to approach him after any quest with the qunari and be honest about what happened. Need three or four instances to be counted for the achievement I think.
