I believe there is a 'canon' version of the game. Certain things that happen in any game are ignored if it does not jive with the 'canon' version.
IE -
1) Leliana will always be alive.
2) Anders will always be alive.
3) Justice will always survive the battle so that he can bond with Anders later.

Oh, I forgot that. I never saw the Orlesian Warden as "my" Warden. It was always my Amell, and as Hawke's cousin she was mentioned often enough to make me forget the Warden-Commander =/
Because the Warden-Commander is no hero, I think. Amell saved Ferelden from the Blight, and this is what they are talking about most of the time, no one mentions the Broodmother....

It's glitched. Some have it, but apparently there's a problem with Witch Hunt.

Actually Anders CAN die - if you let him die in Awakening and bring him with you to the deep roads Nathaniel will comment on his "death". Anders replies that the mage they found dead wasn't him but he used his body as a decoy so he could escape, or something like that.
Have a link, this is what happens.
Perhaps they will add a similar conversation for Leliana, like she was found injured and rescued or something similar.
