I'd say story telling improved but pretty much of the rest got screwed up:
Leveldesign
Userinterface
Inventory
Lowpoly/low texture NPC's
DX 11 probs
Autoquests with pointers on maps
NPC's have nothing to say to each other but are gesturing like they have heated discussions
Copy & Paste Animations sometimes really inappropiate ones
EULA changes

I am not through the game actually and am rather lazy about backing up my claims of screwed-upness with youtube videos but I do have Fraps on whenever I play to fetch those hilarious moments...

Down with the Chantry convert to the Qun or fear them consequences

no, it can't live up to what's nearly perfect -- yet that doesn't make it a bad game, it just can't hold up to the high standard Origins set.

Bioware had some good ideas, but went a bit too far.

Bioware simply did the same thing to Dragon Age II as they did to Mass Effect 2, stripped and simplified. But then again, that won't stop me from buying ME3 or DA3.
