so i though i did everything before i beat the game, but apparently i missed the achievment to kill an undying terror and meet secretly with and agent of the divine, and where the F uck is Flemith? she just goes away after act one?

You didn't unlock the achievement to meet with an agent of the Divine because it's one of the DLC achievements, and just about none of the DLC achievements are unlocking at the moment. Bioware is already aware of the issue and are working to resolve it.
