Hawke told Isabela at one point that Hawke would run off with her and be her first officer or whatever. They did it the one time which concluded with it being no feelings just sex but that and some flirting later was it.

Anders moved in with Hawke & Anders got the armor change, Hawke told him that I would Hawke would run away with Anders, which got me the achievement for completing a romance.

Yet Varric tells Cassandra that all the companions fled the city with Hawke and that circumstances forced them all to abandon Hawke except Isabela. Why did the game chose Isabella a casual sexual thing over Anders the guy Hawke was living with?

It's a game glitch. And maybe Isabella figured she'd meet more men that way.
