First you need to calm down. Second, are you sure you dont use the wrong sheet? There should be two sheet of paper. One with two codes on each side and one with only one code on. You need to redeem both Black Emporium and the Prince Vale DLC on xbox lilve' redeem section.

First time redeeming the code on this site i went to the "dragon age 2 - redeem code" section, put in all the digit (The - too between the codes) and then i got an error saying something about "can't contact to the server". So i typed in the excact same URL that was on the paper, typed in everything again and then i got a green successful screen. What i'm saying is, have you tried to used different mirrors? (I doubt it has something to say, but it worked for me, or at that time anyway), and have you tried to type them in several times? (seeing like the 6 looks very alike the G on the paper). Or maybe you forgot to put in the " - " between the codes.

I followed the step by step directions and I got all my SE edition content, I think too many people are getting in a rush and typing the wrong codes in the wrong places

I always register the game first to my account then type in the codes after

There are M A N Y people posting here that are plenty smart enough to enter their codes. EA has told me that there is an "issue that we need to investigate further" and have stopped communicating with me since then (48 hours now). There is an issue and God knows when it will be resolved. If forced to guess, I would say that it will be resolved about 10 minutes before we get any official communication stating that there is an issue. God forbid people be given information to prevent them from making the issue 100 times more complicated by the time a fix arrives.
