I'm having trouble with the upgrades, more specifically I know for a fact I bought them from the store, but the companions don't have the upgrades applied at the end of Act II/start of Act III.

Fenris has no upgrades whatsoever, Varric only has the one you get in his Act II quest, and the other characters are missing 1/2 each.

I bought all of the upgrades in Act II at the beginning of it, then double checked everything at the end just to make sure that I had them all and the store wasn't selling the upgrades, yet the companions didn't have the upgrades on their armor.  What gives?  Do they have to be present in the party when you buy the upgrade for it to actually take effect?  If so I effectively screwed myself out of a trophy because everyone is missing at least one upgrade.

Im having same issue, I remember buying/obtaining most of these, but they are not on the character.
I figured I maybe went back a save or something so I went back to the vendor (like robes in hightown) and they're not selling them...

Yeah, my game is definitely glitched. I just killed the monster for Fenris' last upgrade, looted it, and it did NOT apply to his armor.

Does anyone know what triggers this?

So far all my quest ones are fine, but everything from Jean Luc disappeared and some from shady merchant and lowtown apparel have.
I sent a ticket to Bioware/EA, hopefully they will have something to say.

I had a similar issue. I obtained both of Fenris rune upgrades, and then before filling the sockets I got his third upgrade. Soon as I obtained his third upgrade (not a rune slot finally) it seemed to make his two rune slots dissapear entirely, and actually said "Obtained item Grafted Skin Hide" or whatever Fenris item is called. After that point, until I received his fourth upgrade, I couldn't add runes.

However, it seemed to fix itself after the fourth upgrade, I gained all his slots back.

i found first varis upgrade at act 1 first merchants on the left just before the tavern.. work for him

I noticed this while on the Justice quest where i knew i bought the first 3 armor pieces for Anders but he only had one.

Someone should probebly post this in the technical forum if it's not there already.
