I didn't get my friendship high with Merrill (only one of few, I think her and Fenris) so I don't have your problem. Once you get with Isabela the first time, you need to convince her about love, I think once you 'investigate' after you sleep with her you get that dialogue option. Then the relationship builds, and eventually she confesses she is falling for you.
It's really one of my favorite romances in a bioware game.

thats saying though she did on mine ask zehran for sex on mine but my character went to her no i do not want to do that and she said and i believe i am quoting now 'OK fine, but we better have make up sex when we get back'
but i did have the same experience as you she she was more afraid of hurting someone else rather her being hurt thats why she ran away when one of her ex's proposed to her.

yes you have to say no to Merrill when she visits you the option is something like she didn't think her clan would approve you say don't want to get them angry at us and she will leave and your back with isabella but if you get in a relationship with her no Isabella isn't a option anymore can't go back i thinkgenocidal villain wrote...
When I started playing DA2 I know my first romance for my character would be Isabela. My first playthrough I was able to flirt with her and eventually Hawke and Isabela slept together. After sleeping with Isabela I know that she was using Hawke, and it was more than lust than romance. After that I would assumed Hawke and Isabela are in a relationship. Later on, I did my companion quest for Merrill and her friendship was high I think it was max too, but she confessed her love to Hawke and we became a couple after that. I don't understand what happened here eventually Isabela told me if Hawke and Merrill are a couple and Hawke said yes, and Isabela said if I do anything wrong to Merrill she would cut my balls. Is there a way to be a in a relationship with Isabela or no?

First playthrough I fling with Isabela then romanced Merril, then I couldn't go on with Isabela.
On my second playthrough, after i slept with Merril i told her it was just a fling, and then I was able to romance Isabela later on in Act 3.

That whole conversation touched me a bit, because she's this big ho, all defensive, nothing but sex, then she drops the whole defensive and pours out the heart.

Yup. Some pretty neat character development right there. Plus the line at the end of Act 3. I was reluctant to do the romance with her at first. At the end I figured it really was impressive. Not what I expected.
