I have fenris at 100% rivalry, did all of his personal quests, but can't get his quest questioning beliefs in act 3. I have it for all the other companions. Is this a bug or did I do something wrong?

Are there two Questioning Beliefs quests for him? I have him at about 3/4 friendship with no gifts, in act 2, and got the quest there. Perhaps it's different for rivalry?

Oh, that might be the problem because i didn't give him a second gift. Thanks. Yes, there are 2 questioning belief quests.

I gave him both gifts and had him at about 75% friendship and never got this quest either. First playthrough I got it no problem, but second it didn't pop.

i got both quests but didn't give the second gift.
in act 2 did you do "a bitter pill" and his 2nd "questioning beliefs" quests?
for the 3rd one to have popped up i think you need to sleep with him and do not break up with him (he will still leave)
after the fight with the arishok you should get companion quests for everyone in act 3, including fenris. you need to kill danarius and afterwards you can kill or spare his sister (doesn't matter which). after that go to his mansion and speak with him for the final "questioning beliefs" quest
