Pretty sure it was retconned since none of the Qunari in DA:O have horns, but I could be wrong.

If I recall correctly, certain branches of the quanari military have horns. Or maybe its certain subtypes of Qunari that have them, sort of like how humans look slightly different from each other depending on race.

Well, I'm actually wondering whether or not what we saw on the Qunari in DAII was their skin or just full body war paint. Though it does make sense, it could be a racial/ethnic thing. Genetically, it's probably a birth defect, but the Qunari see it as a sign of a great destiny. Then again, it's a fantasy worlds, so it could not be a birth defect. Unless they release the science behind the Qunari DNA, we'll never really know
As for skin tone, might just be the group they are from. I mean, humans have variant skin colors, why not Qunari?

Not sure why Sten's skin is that color. Even in Origins they talk about Qunari having gray skin.
