After beating the game the first time, I decided to play through a second time. Upon completing the game for a second time I did not receive credit for the "Epic" achievement. 

This seems to be common with people who did not import a DA:O save (mine was lost when I purchased a new console, unfortunately). On other forums, I have seen posts from people who beat the game THREE times and did not get the Epic achievement. It does seem to work correctly for those who imported a save from the previous game, however.

If it makes any difference, both playthroughs were as a male rogue, but both had different names, appearances, and plot choices.

i have the same problem and i was one of the people that beat the game three times and still did not get it....
