I think my favorite/least favorite will depend on how I play the game and who I take as my companions. For the first time round I kept Anders, Aveline and Varric in my party so I felt like I got to know them a bit better than the others. I was also a female mage and romanced Anders, which definitely makes me a tad biased.
Anyway, based on my first time through:
Favorite: Anders (although Varric is a very close second)
Least Favorite: Carver, and maybe Sebastian although I didn't really see much of him.
I thought Anders was alright in Awakening, but he was not my favorite companion there. However, the new darker and more serious side of Anders, combined with his love of cats and strong desire to help apostate mages, totally won me over in DA2. I can't say I like him better than Alistair yet, but he is an extremely intriguing character for me.
As for Carver as the least favorite, that was mainly because he whined at me too much. But to be fair, I really didn't actually dislike any of the companion characters. They were all interesting in one way or another.

Favorite: Varric. He really did grow on me, and I love using him as a persuade option.
Close Second: Aveline. A character that could have been the classic boring stick in the mud, but instead is a very human, even lovable stick in the mud. And as silly as it was, her romance side quest made me giggle.
Least: Pirate Barbie. Her character model represents pretty much everything that's wrong with the depiction of women in videogames, and the tee-hee sex jokes were old before she even said them. I honestly feel like she pranced in from some other game. Current run, I didn't even talk to her at the bar.

Hard to say. Verric is my favourite, he is awesome with no annoying features. My least favourite would have to be Isabella, I like nothing about her character. Merril and Fenris was also annoying. I initially liked Fenris, but he became too whiny and narrow-minded.

Favorite: Easy, and by a large margin--Varric

Although I will say Anders provides some shock and awe which at least tries to make up for how much of a whiny dip he is; I'll also say his character concept was interesting, just not well executed. As well, Aveline's companion quest where you play cupid was good for several laughs at least.

Favorite: Isabella. I don't see why there is so much hate against her. Some people say she's shallow and skanky. I guess you haven't seen her good side. If you treat her well she is a very compassionate and caring person, in her own way. She is still quite scarred due to her past experiences, but then again, so are many real people... Only negative, I'd prefer if she had smaller boobs, but that's just my personal preference 
Varric and Aveline are also awesome characters that I really like.
These three are the most "life-like" and well done IMO. Bethany gets a honorable mention, but I wish she was more fleshed out.
The others I'm not so fond of. Especially not Fenric, I think he's a bit too single-minded. The same is true for Merric and Anders IMO.

Fourth favorite? Biscuit, the amazing teleporting mabari!
And Aveline and Fenris are up there, too.

I like dwarves, and Varric is the best dwarf. He's like a not totally drunk version of Oghren that's less inappropriate and more useful in a fight. He also has a Five-O'clock Shadow.
Aveline was the perfect character to side with my GoodMaleHawke. They were both good guys, working to help people... albeit with weird methods. She was also so awkward in the companion quest with Donnic, and had some funny banters and conversations. She also sky-rocketed up when I did the quest with Leandra's death -- since I hadn't romanced Merrill yet -- being the only person that asked to talk about it.
Also... *cough*gavemethefinger*cough*
I can't say much about Fenris...
Oh wait... I can. A broody little elf. White hair, emo hair and skin-tight clothes. He hates mages because ONE was an **** to him, not even because he was a mage. He was bad in a fight compared to other characters (namely Varric, Isabela, Merrill, and Anders).
I really dislike what BioWare did with him. I had a two-hander using Dalish elf in DA:O (WITH corn-rows), that I made a total bad-ass. I regretted it when I got Fenris in the group.........
Maybe if he had normal hair, wasn't broody, and didn't despise mages, I'd like him more. The character concept and voice were good, but the use was...............

The conversations these two have are so very amusing, and I genuinely like their characters. But if he counts, Mabari = my absolute favorite.
Least Favorite: Anders...
While I can appreciate his character, that doesn't mean it's easy for me to tolerate him... When granted the chance to end his whining and putting him out of my entourage's misery I took it without hesitation. The only regret I have is making a martyr out of him...
Perhaps I'm being a bit too harsh, but I just couldn't stand his childish tantrums and his inability to own up to his own shortcomings (using Justice as a scapegoat all the way up until the final act, iirc)...
Personally, I'm opposed to the Templars, but this guy... he just had to go.

Least favourite - this one is hard... I thought it would be Isabella but she made me laugh a lot in her banter and the game needed some laughs. I don't really think I had a least favourite except maybe Sebastian because I felt his character was rushed and didn't really feel finished or part of the game properly.

Least fave: Fenris; i just couldn't find a reason to bring him along for more than an hour or so. Very boring character.
Honorary mention to Varric and Aveline who were both great. Bethany would be also very high on my list, should i'd gotten more screentime with her.
