Question about the Fade (Act 2 spoilers)

Merril turned on me, so I had to kill her in the Fade.  This should have led to her becoming Tranquil in the real world.  Why didn't Bioware go this route when everything else in the game states this is what should have happened?  I mean five minutes after this happened Feynriel is begging me to kill him and Hawke tells him that if he dies in the Fade he'll be made Tranquil in the real world, but my party members just disappear in a plume of smoke and apologize later?

because that's never been stated. Killing a Mage in his Dream destroys his connection to the Fade. You are not in Merrils Dream, killing her in another Mages Dream, causes her to wake up.

You didn't use the murder knife, that's why.
