When and ogre charged Hawke's family while they were escaping during the opening part of the game. Bethany was trampled and killed.  Now I see on the posts that Bethany must have survived or else I have missed something (which is probably the case as that is my typical modus operendi)

Bethany is only killed if you play as a mage. If you are a warrior or rogue, Carver is killed instead.
