Just finished the game (awesome, awesome game!) and...... well the title sais it all :] she never made an appearance LOL anyone else have this happen ?
Completed DA2 but i never met Isabella ?

I think she randomly appears at some point when you enter the Hanged Man in act one, so it seems entirely possible that you could never have that happen. I'm not sure act two would go at that point though...

The devs said it is indeed possible to "miss" companions as in DA:O. To get Isabella, you need to visit the Hanged Man in Lowtown at night; a cutscene should trigger with her beating up some thugs and she'll give you a quest.

No but I have no idea when it´s supposed to trigger, first time through I was at level 12 the second it was my first night trip to the hanged man, roughly an hour into the first act. I know it´s nightime at the hanged man but not what triggers it.

She appears in The Hanged Man after you do Anders' first quest. It's essentially impossible to miss her...

ill make sure to not miss her in my second play through ;]............... felt a bit weird at the end when Cassandra the seeker is looking through the book with the companions faces and Isabella's is at the top LOL

Well no matter what you do she is related to the whole thing through the relic... I don't know how that specific plot turns out if you never meet her though...

I have Isabella, but somehow managed to miss Fenris, and I have no idea how it happened either! I guess a second play through for me is a must as well.

you meet fenris through letter mission, and isabella half way in act 1 in the hanged man where she fights off, yet again, 3 dumb men

missing isabella in DA2 is like when people miss leliana or sten in DA:O

Both Fenris and Isabella are optional so to speak. They don't join your party automatically, you have to go hunt them down. Which is stupid regarding Isabella's contribution to Act 2.

You'll also miss Ambrosia if you don't complete her quest line, which can also occur if she books it and doesn't come back (****).

Ambrosia? Seems I missed quite a bit.
As for Isabella, the Incident with the Qunari can end in only one way if you miss her/don't do her quest.
