 Where is the UPC code on the box? I have entered every GD number on this stupid box.

Usually bottom right corner, it should be 12 or 13 digits.

What if you bought the game off the PSN...how do I register it?

I entered the 12-digit UPC from my box and the site claims that it's not a valid number. I've triple-checked (twice) that what I entered was correct. There are two smaller numbers to either side of the UPC. Are those part of the code (which would make it 14 digits)?

Send a PM with your code to Jeff Roussell (or possibly Jesse.Reid, I don't know). Sometimes the UPC codes are unrecognized, at least, that was a problem with some of the DA:O Ultimate Edition cases. Jeff's a forum administrator who should be able to sort it out for you.
