If the Hero of Fereldan lived they are the Warden Commander. If the Hero of Fereldan died the Warden Commander is the Orlesian Warden. I thought that was quite straight forward. This way they (the writers) can reference the events of Origins and Awakening and use the different titles but still refer to the same person (if alive) without having to record multiple dialog line to reflect all the differeing possibilities.
