The biggest failure in my opinion was the mandatory death of either Bethany or Carver. Disregarding how cheap (but I'll admit, still sad) their deaths are, both should have survived the opening act. If Bioware really wanted to kill a family member during the escape, it should have been the father. Instead of dying before the game begins, he should have died protecting his wife and children. That way you'll get a better feel of the Hawke family and it saves Bethany and Carver for the events later in the game, not to mention, makes Malcolm fell more significant.
Now think, how much more interesting would the rest of the game be if Carver joined the Templars and either ratted out Bethany, or the Templars have learned of her location themselves. In the last act, I of course sided with mages since my sister was among them, but would have Carver been with the Templars, it would have given me a pause. Sure, he's an ass. But he's MY ass of a brother. I'd still probably side with Bethany, but It won't be an easy decision.
That said, more interaction with both siblings following act 1 would have made the game much more enjoyable to me. You get one letter in act 2 and that's it (not counting the part if they become Gray Wardens) until you meet them at the end of act 3. I mean how much pain would it be to give us at least a few moments to visit our siblings at the Gallows (if they're not Wardens) and talk to them? Especially once Leandra died, I really felt the urge to find my sister and talk to her about it. Or have them ask about how you're doing and wonder about whomever you romanced. You know, stuff like that. I had hoped that there would be some time to catch up in act 3, but Bioware had to go and trivialize Bethany's and Carver's appearances there as well.
For a more personal story, it sure felt underwhelming, and I sincerely hope Bioware would look into it more seriously.
