What can be done, or did I miss something big already covered?

I do as well... I hope that doesn't have anything to do with this.

I don't think so. I don't think the patch is out for steam yet, where I bought the game. I'm having this bug too. It's weird, cause it doesn't seem like a lot of people do...

Well lets hope it's just not a Steam issue because I don't play via Steam

Turn Subtitles to Always On.
This won't fix the audio, but at least you know what was said.

Actually, it doesn't help. The text won't show either.
Beside these cases of just the dialogue wheel showing up, it seems to randomly affect the first node of all dialogues. You can often see party banters start with the first reply instead of the beginning, and it's very hard to get the game to show/play dialogues that consist of only one node, like Hawke examining some objects in their house, or talking to party members when they'd respond with only one line.
No idea of the cause or a solution. In an other thread someone speculated that it might happen when overusing the highlight objects -key, but leaving it unused didn't help with me.

That's what is bugging me the most. I keep thinking I'm missing something important or that I'm going to pick the wrong reply and mess something up.

I suddenly have no problems with this anymore, but I do not know why... I haven't done anything. Maybe a patch has sneeked in thru steam, but I don't know... I'm not accustomed to how steam works yet, but I know it does things automatically alot, which can be annoying because I have no idea what's going on, and more often than not, it messes up modded games. But in this case it might have help though...
Nope, now it's back again. And now things were going so well

Anyway, I read somewhere that if you go into options, then audio and check "force stereo sound", it might help the issue. I don't think it helped me; when I went into options, that box was already checked. I unchecked & rechecked it, but I still need to see if it actually worked. At any rate, maybe it'll work for someone. :-)

It affects almost every conversation cut-scene and the ambient dialogue as well.
Are you sure? With me it has always been somehow related to the party members or Hawke, ie. if the dialogue would begin with their animation or line, it won't show, regardless of who the rest of the conversation is with.
Yet it happens even if they aren't currently members of the party, in their homes, for example.
