I had the problem before and after I installed the patch.
What happens to me is it'll cut straight to the the dialogue wheel or sometimes it will eliminate some of the beginning phrases giving me only part of the beginning conversation. E.g. in the beginning when you meet Wesley and Aveline it skips straight to the "The order dictates..." line. But it doesn't happen every time. Before the patch I played the beginning sequence like 7 times, hoping to get past the loading screen, and this happened about 3 or 4 times.
When I try to examine things in the home it'll skip the first line, and I'm assuming it won't tell me the one liners because I can't get them to play. Sometimes I can get some of them to work though.
When my companions talk to each other in party, I don't get the first line. It'll skip straight to the response. I'll get the random conversation that works sometimes.
It's very odd.

Well that's great for you, but that's not the problem we're having. In our case, the first few lines of dialogue are being completely skipped, and then the rest of the conversation continues as normal. When I say skipped, I mean not just the audio, but the text and animation that accompanies the line are missing as well. We just get a conversation wheel out of the blue.frogemoth wrote...

After I selected the option Force Surround Sound the problem has gone away, had to restart DA2 though for this to work.

I noticed this also, from the prologue i didn't see the cut scene of Bethany saying 'where are we going?' (or similar) and it goes straight to the dialogue wheel. I have also noticed it also with some of the other companions scenes. Weirder still though is that its only with my Mage characters... the rogue and warrior see the cut scenes from the start. I created a second mage but the same thing happens.
Yes, all of the conversations. I know this because I can play it on another computer and and I can recognize the missing dialogue.Terendil wrote...

Are you sure? With me it has always been somehow related to the party members or Hawke, ie. if the dialogue would begin with their animation or line, it won't show, regardless of who the rest of the conversation is with.
Yet it happens even if they aren't currently members of the party, in their homes, for example.

I had this issue through my entire first playthrough. Wasn't able to find any fixes, so I just sucked up missing the first bit of most of the important scenes.
Just to reiterate, the problems I'm having are:
1. Cut scenes and conversations with NPC's skip over all dialogue until the first conversation wheel. No way to see what was said, even if subtitles are on (also... I missed Fenris nookie. Had to go watch it on youtube to see what I'd missed.)
2. Incidental street conversations between NPC companions start at second line (response) of dialogue.
3. Single node things (as when I examine things in my house) produce no dialogue, except on the rare occasion when one of the objects has a second node.
4. On rare occasions, I can reload from a save just before the conversation in order to get the entire thing. I usually have to try this 4-5 times before it works.
5. I had the single-core save issue, downloaded the patch right away (otherwise... there wouldn't have been a playthrough to experience problems during).
