I am in my second play through and have encountered the same import related issues.
- I received the Act II quest Fool's Gold due to the fact it believes Nathanial is dead(Confirmed when his Sister was not present in Act III)
- I did not receive the Act I quest involving the Silver Order
- Leliana does not acknowledge the romance from Origins
Dragon Age: Origins Import information:
- Used the post game save from Witch Hunt.
- Upgraded the Keep completely in Awakening
- Left Nathanial, Anders, and Justice to defend the keep
- Gave Master Wade all the metals and completed his side projects in Awakening
- Don't
know if this may have bugged it but I did romance Morrigan and Lelianna
at the same time (after necessary break-ups, new gifts, rekindling etc.)
